摘要
【目的】观察补肾健脾中药复方助孕丸对复发性流产患者子宫内膜容受性的干预作用。【方法】选择中医辨证为脾肾亏虚型复发性流产患者10例为治疗组(服用助孕丸治疗),另选择月经规则、有正常生育史的妇女10例为正常对照组(正常组),比较2组围着床期子宫内膜厚度及子宫内膜下螺旋动脉血流阻力指数(RI)、搏动指数(PI)以及血清雌二醇(E2)、孕酮(P)含量变化情况。【结果】与正常组比较,治疗组治疗前围着床期子宫内膜厚度减少(P<0.05),而RI增高(P<0.05),血清E2含量及E2/P比值升高(P<0.05)。2组间PI、血清P含量比较差异无显著性意义(P>0.05)。与治疗前比较,治疗组治疗1个月后,RI指数降低(P<0.05);治疗2个月后,子宫内膜厚度增加(P<0.05),血清P含量显著升高(P<0.05),E2/P比值呈下降趋势(与治疗前比较,P>0.05)。【结论】子宫内膜厚度、RI、E2/P比值可能是影响子宫内膜容受性的重要因素。助孕丸能够有效增加脾肾亏虚型复发性流产患者子宫内膜厚度,降低RI,改善子宫内膜容受性。
Objective To investigate the therapeutic effect of Zhuyun Pill, which has the actions of tonifying kidney and strengthening spleen, on endometrial receptivity of recurrent abortion. Methods Ten recurrent abortion patients with deficiency of spleen and kidney in the treatment group were given oral use of Zhuyun Pill. Ten healthy volunteers with normal childbearing history served as the normal control. We compared the changes of endometlial thickness, resistance index （RI） and pulse index （PI） of endometrial spiral arteries, and serum estrogen （E2） and progesterone （P） levels. Results Compared with the normal control group, the endometrial thickness during peri-nidation stage was reduced, and RI, serum E2 content and the ratio of E2/P were increased in the treatment group （P 〈 0.05 ）. The differences of PI and serum P content were insignificant （P 〉 0. 05）. Compared with those before treatment, RI in the treatment group was decreased one month after treatment （P 〈 0. 05）, and the endometrial thickness and serum P content were increased （P 〈 0. 05 ） and E2/P ratio was decreased （P 〉 0. 05） 2 months after treatment. Conclusion The endometrial thickness, RI and E2/P ratio probably are the influencing factors of endometrial receptivity. The possible mechanism of Zhuyun Pill is related with the improvement of endometrial receptivity of recurrent abortion through increasing the endometrial thickness and decreasing RI.
