土茯苓加减方对淋病奈瑟菌耐药质粒的消除作用 被引量:4

摘要 目的评价土茯苓加减方对淋病奈瑟菌耐药质粒的消除作用,为筛选安全、有效、经济的中药消质剂提供参考。方法四川省医学科学院.四川省人民医院皮肤病性病防治研究所性传播疾病(STD)实验室2005年2~12月收集的门诊淋病患者中分离鉴定的30株产β-内酰胺酶的淋病奈瑟菌菌株(PPNG)作为实验菌株(土茯苓加减方组),采用土茯苓加减方对PPNG进行耐药质粒消除;全国性病中心提供的WHO推荐淋病奈瑟菌标准菌株A、B、C、D、E作为质控菌株,采用传统消质剂十二烷基硫酸钠(SDS)作为对照(SDS组),评价PPNG经土茯苓加减方作用后对抗生素的敏感性改变情况以及土茯苓加减方的消质作用。结果土茯苓加减方组PPNG对青霉素、四环素、环丙沙星的耐药率由100%降低为60%~70%,差异均有统计学意义(P<0.01);SDS组PPNG对青霉素和环丙沙星的耐药率下降为77%和80%(P值分别为0.024和0.011);土茯苓加减方对4.2、5.3和7.4kb质粒有不同程度消除作用,其中对5.3kb和7.4kb质粒的消除率达到20%左右;土茯苓加减方组总的质粒消除率为16.7%,SDS组总的质粒消除率为23.3%,但差异无统计学意义(P>0.05)。结论土茯苓加减方对产β-内酰胺酶的淋病奈瑟菌耐药质粒具有一定消除作用,能够在一定程度上增强淋病奈瑟菌对抗生素的敏感性。 Objective To explore and evaluate the effect of Chinese herbal formula on drug-resistant plasmid of Neisseria gonorrhoeae,try to provide a safe,effective and economic formula for plasmid elimination.Methods Thirty experimental strains (Glabrous Greenbrier Rhizome group),Neisseria gonorrhoeae strains (PPNG) produced β-lactamase,were collected from clinic patients with gonorrhea in sexually transmitted disease (STD) laboratory of Institute of Dermatology and Venereology,Sichuan Academy of Medical Sciences and Sichuan Provincial People's Hospital,from Feb to Dec 2005.We used Glabrous Greenbrier Rhizome formula to eliminate the drug-resistant plasmid of PPNG.Sodium dodecyl sulfate (SDS) was used on strain A,B,C,D,E,WHO recommended standard PPNG strains from National Center for STD Control,as a control (SDS group).We evaluated change of PPNG's sensitivity to antibiotic and effect of Glabrous Greenbrier Rhizome formula.Results In Glabrous Greenbrier Rhizome group,incidence of PPNG resistant to penicillin,tetracycline and ciprofloxacin reduced significantly from 100% to 60%~70% (P 0.01).In SDS group,incidence of PPNG resistant to penicillin and ciprofloxacin reduced significantly to 77% and 80% respectively (P 0.05).Glabrous Greenbrier Rhizome formula could eliminate 4.2 kb,5.3 kb and 7.4 kb plasmid in different degrees.The elimination coefficient to 5.3 kb and 7.4 kb plasmid was about 20%.The total elimination coefficient in Glabrous Greenbrier Rhizome group and SDS group were 16.7% and 23.3% respectively.There's no significant difference between the two groups (P 0.05).Conclusion Glabrous Greenbrier Rhizome formula has certain effect on eliminating drug-resistance plasmid,which can enhance the antibiotics-resistance of PPNG.
