摘要
目的探讨ARROW中心静脉导管在胸腔积液引流患者中的应用效果及护理体会。方法回顾性分析42例应用ARROW中心静脉导管进行胸腔引流患者的临床资料,并总结护理要点。结果 42例患者均1次置管成功,2例脓胸患者在置管后第2d出现导管堵塞,后改为传统胸腔闭式引流术。其余40例患者导管留置时间为2-11d,未发生导管相关性感染、导管阻塞及导管脱出等并发症。结论应用ARROW中心静脉导管引流胸腔积液,具有操作方便、患者体位舒适、对置管局部刺激少等优点;严格无菌穿刺,术后保持引流通畅、固定良好、密切观察引流液颜色及量的变化、及时处理并发症是保证引流成功的关键。
Objective To evaluate the effects of ARROW catheter for patients with plural effusion and to summarize nursing experiences.Methods Forty-two patients with plural effusion were treated with application of ARROW catheter.Clinical materials were retrospectively analyzed and the key points of nursing were summarized.Results Operations were successful in all patients.Clog of catheter occurred and were transferred to conventional closed thorax drainage in two patients with empyema at the second day of ARROW catheter insertion.The rest of the catheters were patent and kept for a range of 2 to 11 days（mean 4.24 d）.The effusion was drained thoroughly through the catheter and symptoms like respiratory difficulty relieved.Lung re-expansion was confirmed by X-ray 2-3 days after catheter insertion.No complications such as catheter-related infection,catheter occlusion and catheter exfoliation occurred.Conclusions The method has such advantages as convenient operation,less irritation and being comfortable to patients.The key points for successful treatment are strict aseptic operation,keeping good fixation and patency of the catheter,close monitoring of the color and volume of drainage,and timely treatment of complications.
