摘要
目的:研究HIF-1α与bcl-2、Bax在胃癌及正常胃组织中表达的相互关系,探讨它们在胃癌的发生发展、浸润和转移中的作用。方法:应用免疫组化SP法检测54例胃癌中HIF-1α、bcl-2、Bax的表达情况,用脱氧核糖核酸末端转移酶介导的dUTP缺口末端标记法(TUNEL)检测胃癌组织的凋亡率。结果:HIF-1α在胃癌中的表达率(74.07%)明显高于正常胃组织(0%)(P<0.01);HIF-1α的表达与胃癌TNM分期、肿瘤浸润深度和淋巴结转移显著相关(P<0.05)。胃癌中bcl-2高表达(64.8%),且表达率在组织学分级高(P=0.035)、无淋巴转移(P=0.032)和临床分期高(P=0.009)时升高。Bax在正常胃组织中表达率(60%)明显高于胃癌组织(40.7%)。胃癌组织中HIF-1α阳性组的细胞凋亡率明显低于阴性组(P<0.05)。胃癌组织中HIF-1α与bcl-2的表达呈正相关(r=0.266,P<0.05),而与Bax的表达呈负相关(r=-0.253,P<0.05)。结论:HIF-1α和bcl-2、Bax在胃癌的发生发展中起重要作用,联合检测可能作为判断胃癌恶性程度和预后的指标。
Objective：To study the expression of HIF-1α,bcl-2 and Bax in gastric cancer and their relationship with tumor angiogenesis,clinicopathologic feature and prognosis.Methods：Immunohistochemical technique was used to detect the expression of HIF-1α,bcl-2 and Bax in 54 cases of gastric cancer.TUNEL-positive cells were counted in these samples as apoptosis index（AI）.Results：The positive expression rate of HIF-1α in gastric cancer（74.07%） was significantly higher than that in normal gastric tissue（0%）（P〈0.01）;The expression of HIF-1α in gastric cancer was significantly associated with TNM stage,invasive depth and lymph-node metastasis（P〈0.05）;The expression of bcl-2 in gastric cancers was high（64.8%）,and increased with histological class（P=0.035）,clinical stages（P=0.009） and lympha node（P=0.032）.The AI in HIF-1α（＋）tumors was significantly lower than that in HIF-lα（-）（P〈0.05）.There was a positive correlation between the expression of HIF-1α and bcl-2（r=0.266,P〈0.05） and a negative correlation between expression of HIF-1α and Bax（r=-0.253,P〈0.05）.Conclusion：HIF-1α,bcl-2 and Bax play very important role in the development in gastric cancer,could be factors in diagnosis of gastric cancer and estimation of prognosis.
