摘要
目的探讨运动心肺功能测定在结缔组织病亚临床肺损害的早期诊断价值。方法采用Jaeger公司产电闸功率自行车,依据ramppatern方案,每2分钟连续递增运动负荷25W,期间采用心电图监测心率和节律。并于运动前后行动脉血气分析及一氧化碳弥散量的测定。结果结缔组织病患者最大摄氧量(VO2max)最大功率(WATT),达到无氧阈时的摄氧量(VO2),氧脉搏(VO2/HR),氧通气当量(VE/VO2AT)均与对照组有明显差异(P<001),运动后一氧化碳弥散量(DLCO)进一步降低,肺泡动脉氧分压差增大〔P(Aa)O2〕。结论结缔组织病患者存在亚临床肺损害,运动心肺功能可早期测得这种改变。
Objective To detect early
features of lung involvement in connective tissue diseases(CTD),patients with CTD were
investigated using cardiopulmonary exercise test. Methods The exercise test was performed in
the subjects using an electronically braked cycle ergometer by the method of ramppattern.The
workload was subsequently increased by 25 W per 2 min,Heart rate and rhythm were monitored
by ECG.Carbon monoxide diffusing capacity (Dco) and arterial blood gas were measured before
and after exercise tests. Results VO2,WATTmax,and VO2,VO2/HR and VE/VO2AT at anaerobic
threshold (AT) all decreased significantly in connective tissue diseases compared with the
control,while a further decreased Dco and widened alveolaralterial oxygen difference were
present after exercise test.Conclusion Subclinical pulmonary impairment may be present in
CTD patients and it can be tested by the cardiopulmonary exercise test.
