摘要
本文用脉冲多普勒测定4~13岁正常儿童和房缺患儿各20名的主、肺动脉血流频谱,经仪器微电脑处理获得主、肺动脉频谱的PFV、A、AT、AT-FVI。根据牛顿定理计算出心室收缩时对血流加速时间内搏出的血液质量所施加的压力F和产生的冲量I来评价心室收缩功能。结果显示正常儿童左室F大于右室F,I无显著性差异。房缺左室F和I较正常儿童小,右室F和Ⅰ较正常儿童大。且F和I测值在正常心脏与病态心脏之间无重叠。表明F和I能精确反映心室收缩功能的变化,是脉冲多普勒检测心功能的理想指标。
Doppler spectra of proximal Aorta(AO) and pulmonary artery(PA) of 20 normal children and 20 children with ASD were recorded and digitized to obtain peak flow velocity(PFV), acceleration(A), acceleration time(AT), flow velocity integrals during AT (AT-FVI) and the cross sectional area(CSA) of AO and PA were got from π·(D / 2)~2. By Newton's law: Force = mass·acceleration, Impulse = Force·time, F and I were calculated from equations F = 1.06· CSA· AT-FVI· A, I = F · AT to assess systolic function of ventricle. The results show F of LV is larger than that of RV(P<0.001) and I is equal between LV and RV in normal children; F and I of LV in ASD is smaller than that of normal children, and F and I of RV in ASD is larger than that of normal children(P<0.001), especially, there is no over lap in F and I range between normal and diseased heart. The study indicates that F and I can assess the change in ventriclar systolic function accurately.
