中药配合卵泡穿刺术治疗多囊卵巢综合征不孕症的临床观察被引量：4

摘要目的:研究妇科千金胶囊配合B超引导下经阴道未成熟卵泡穿刺治疗耐克罗米芬的多囊卵巢综合征(PCOS)不孕症患者的临床疗效,探讨其对患者内分泌及基础窦卵泡数、C-反应蛋白水平的影响,并观察其治疗后的卵巢排卵功能,以及部分患者治疗后用克罗米芬(CC)或促性腺激素(HMG)促排卵的反应性及妊娠情况。方法:对30例耐克罗米芬PCOS患者用CC/HMG促排卵后,当优势卵泡直径达到12-14 mm时,肌注HCG 5000-10000 U,32-36 h在超声引导下经阴道卵泡穿刺治疗,同时口服妇科千金胶囊,连用2-3个周期,检查穿刺后患者的内分泌、卵巢基础窦卵泡计数和排卵情况以及C-反应蛋白的变化,并随访其有无妊娠。结果:对30例耐克罗米芬PCOS患者进行了71个周期的穿刺治疗,患者睾酮(T)和促黄体生成素/促卵泡激素(LH/FSH)比值均明显降低,与治疗前比较,差异有统计学意义(P<0.01),基础窦卵泡计数降到10个/卵巢以下;术后监测排卵,20例(66.7%)恢复排卵;12例(40.0%)临床妊娠,无1例发生卵巢过度刺激综合征(OHSS)。结论:对难治型PCOS患者应用妇科千金胶囊配合超声引导下经阴道未成熟卵泡穿刺技术可改善其内分泌异常及胰岛素抵抗状态,减少基础窦卵泡计数,恢复排卵功能而妊娠。
