摘要
目的针刺功能性消化不良(functional dyspepsia,FD)患者足三里穴(ST36),观察其胃排空的变化以及与胃排空相关激素水平的变化,为临床治疗提供依据。方法根据罗马Ⅲ标准,确定FD患者。采用B超预先测定FD患者进餐前、后胃排空情况。采用针刺FD患者双侧足三里穴(连续3d),再测定进餐前、后其胃排空情况,同步测定与胃运动相关的胃肠道激素胃动素(MTL)、生长抑素(SS)以及观察针刺FD患者足三里穴对其胃动力作用的影响。同时选择健康志愿为对照组。结果 FD患者在进餐后,其胃排空呈现减慢趋势,胃肠道激素水平与正常对照组相比,有统计学差异。而给予针刺足三里后,FD患者胃排空较未针刺状况相比明显加快,接近于正常对照组。FD患者MTL、SS水平在针刺后均较针刺前为高。结论针刺FD患者足三里,可以改善FD患者胃排空障碍,其主要变化可能与某些胃肠运动相关的激素水平变化相关。
Objective To investigate the changes of gastric emptying and its relations with the changes of hormone levels after acupuncture at Zusanli（ST36）in functional dyspepsia（FD）patients for providing a basis for the clinical treatment.Methods The gastric emptying changes in FD patients diagnosed by Rome Ⅲ criteria were predetermined by B-ultrasonography before and after meals.The gastric motility and gastrointestinal hormone-related motilin（MTL）and somatostatin（SS）were simultaneously determined for 3 d before and after the meal after acupuncture at Zusanli（ST36）,and the effects of acupuncture at Zusanli on regulating gastric motility was observed.The healthy volunteer individuals were chosen as controls（control group）.Results The gastric emptying of FD patients showed the trend of decrease after the meal,and there was significant difference in gastrointestinal hormones between FD patients and controls.But the gastric emptying increased after acupuncture at Zusanli in patients with FD,which was close to the normal controls.The levels of MTL in patients with FD increased after acupuncture,while SS levels decreased.Conclusion Acupuncture at Zusanli can increase the gastric emptying in FD patients,and the main changes of gastrointestinal motility may be related with changes of some related hormones in patients with FD.
