Reconstruction of Temporomandibular Joint.

摘要 为了探讨颞下颌关节真性强直的外科治疗,解决术后易复发的难题,作者近十年来采用自体肋骨肋软骨游离移植和/或下颌骨升支后缘纵截倒置,对38例颞下颌关节真性强直患者进行了关节重建术,术后随访,测量张口度,计算出随诊时张口度减术前张口度的差值,检查咬关系,下颌前伸和侧向运动功能,并通过关节开闭口位X线片来分析观察关节间隙,从而综合评价手术治疗效果。结果发现,37例患者术后张口度的差值在2.0cm以上,咬关系好,下颌具有前伸和侧向运动功能,重建的关节具有正常的关节间隙,功能和外形好,表明自体肋骨肋软骨游离移植和/或下颌骨升支后缘纵截倒置的颞下颌关节重建术是治疗颞下颌关节真性强直的一种理想方法。 Objective: To study the surgical treatment of ankylosed condyles. Methodsa:Autogenous costochondral grafts and/or invert replantation of posteior segment of ramus were applied in the reconstruction of the temporomendibular joint. In 38 cases with ankylosed condyles from 1988 ̄1997. The follow up period was 1 ̄10 years. The degree of mouth opening pre and post operative was taken, The evaluation of the results of the surgical treatment was based on the TMJ function. Result:37 cases had their degree of mouth opening post operative improved up to 2cm. The authors achieved normal TMJ function in 97.4% cases. The authors and the patients were satisfied with the treatment results. Conclusion:The use of costochondral grafts and/or invert replantation of posteior segment of ramus in TMJ reconstruction is the best approach in the treatment of TMJ ankylosis.
