摘要
目的:探讨带拔牙创传送盘行牵引成骨修复犬下颌骨节段缺损的可行性。方法:选用8只本地杂种成年犬随机分为A、B两组,每组各4只。分别采用自行研制的两款牵引器,构建带右下颌第一磨牙拔牙创的传送盘行传送盘牵引成骨修复右下颌骨前磨牙区节段缺失的杂种犬动物模型,同时拔除对侧第一磨牙做为对照。固定期8周后处死取材,观察牵张区新骨生成情况,传送盘改建情况及对侧第一磨牙(M1)拔牙创愈合情况。结果:B组犬在间歇期先后出现传送盘感染坏死。A组4只犬顺利完成牵引,固定8周的组织学观察证实牵张区新骨成熟,传送盘表现为广泛活跃的膜内成骨过程,对侧拔牙创愈合良好。结论:带拔牙创的传送盘牵引成骨修复下颌骨节段缺损是可行的;传送盘与牵引器固定钛钉应避免打入拔牙创内,否则可导致传送盘感染坏死。
Objective: To certify whether the transport disc with:tooth extraction wound can reconstruct the seg- mental mandibular defects. Methods: Eight dogs were randomly divided into two groups. Type " I " and "T"distractors were separately applied to group A and group B. The mandibular defects were made by the resection from the second premolar to the fourth premolar in the right mandibular, at the same time, we pulled out the first molar in the two sides of mandible. The tooth extraction of left mandible wound was served as the control. The transport disc was completed between the medial trump. Dogs were sacrificed at 8 weeks after the completion of distraction. Result: In the latency, the transporting disc of group B were infected and necrotic. Distration osteogenesis in group A was successful. Compared with the control side, the transport ciisc develop a widely intramembranous ossifica- tion, and less bone was regenerated. Conclusion: It is feasible that the mandibular segmental defects were reconstructed by the transport distraction osteogenesis of a transport disc with tooth extraction wound. The position of the screws which fixed the transport disc should avoid the extration sockets. If not, the transport disc would be infected and necrotic.
