种植体—杆卡式附着体固位的覆盖全口义齿设计 被引量:4

摘要 〕目的:总结种植体—杆卡式附着体固位的覆盖全口义齿的临床设计经验。方法:随访观察46例种植体—杆卡式附着体固位的覆盖全口义齿的修复效果,评价杆式支架的不同设计形式和位置对修复效果的影响。结果:分别采用了三类不同杆式支架设计的义齿均获得了良好的固位和支持;杆与牙槽嵴粘膜间距过小或位置异常可引起局部粘膜炎症及影响口腔功能。结论:应根据患者的牙槽嵴和颌弓条件分别选用三类不同的杆设计形式;杆与牙槽嵴粘膜间距应保持在2mm以上;杆式支架应位于牙槽嵴顶上方且与双侧颞下颌关节转动轴平行。 Objective: To sum up the clinical experience in the design of complete overdenture retained with implants and bar clip attachments.Method:46 cases with complete overdentures retained with implants and bar clip attachments were followed up for 12~36 months and the influences of differently designed bars to the restorative effects of the denture were evaluated.Result: A11 complete overdentures with 3 types of differently designed bars obtained excellent retentive and supporting effects.The local inflammation and oral dysfunction was observed in the cases with minor space between bar and alveolar mucosa and abnormal position of the bar.Conclusion:Bar should be designed according to the alveolar ridge and maxillary or mandibular arch of the patients and the principle of Spiekermann's design.The space between bar and alveolar mucosa should be kept more than 2 mm;the bar should be above alveolar ridge and parallel to the revolve axle of temporomandibular joints.
