摘要
从病因、证候、胃镜下表现对胃溃疡及十二指肠溃疡进行分析比较,二者既有相同点又存在差异,通过临床及研究发现,胃溃疡与十二指肠溃疡活动期均为热毒血瘀致病,但中医病机又具有本质的区别。胃溃疡病机为热毒血瘀,脾失健运,证候表现上以毒热致瘀为主。十二指肠溃疡病机为脾虚血瘀,寒郁生火,证候表现上以虚寒致瘀为本。故治疗上应分别施治加以区别对待,故引发了中医辨病论治的思考,根据两种疾病病机本质的区别分别进行辨证论治。
This paper does analysis and comparison of gastric uncle （GU） and duodenal uncle （DU） on etiology, syndrome and electronic gastroscope. GU and DU have some similarities and deference. By clinical and research found both GU and DU in active stage are from heat toxin stasis in blood, but their pathogenesis are essentially different of Traditional Chinese Medicine. Pathogenesis of gastric uncle is heat toxin stasis in blood and spleen failing in transportation, primary representation of syndrome is heat toxin stasis. Pathogenesis of duodenal uncle is spleen deficiency with blood stasis and cold stasis engenders fire, primary representation of syndrome is deficiency cold stasis. So GU and DU treatments are difference. Therefore disease identification and treatment of TCM must be considering.Syndrome identification and treatment of GU and DU is base on their essential differences of pathogenesis.
