血液灌流联合环磷酰胺救治百草枯中毒临床分析 被引量:18

摘要 目的探讨血液灌流联合环磷酰胺对急性百草枯中毒的疗效。方法该研究对我院2007-07~2010-02确诊的急性百草枯中毒患者病历资料进行回顾性分析,将患者分成三组,A组为常规治疗,B组为常规治疗加血液灌流治疗,C组为常规治疗加血液灌流联合环磷酰胺治疗。比较各组患者肝肾功能、中毒性心肌炎和MODS发生率及死亡率等变化。三组间均数比较采用方差分析或秩和检验,两两比较采用LSD法。多个率比较采用非校正矿检验。结果该研究入选患者104例,三组患者年龄、性别及服药剂量分布差异无统计学意义(P〉0.05)。C组患者的肝肾功能、中毒性心肌炎发生率及死亡病例存活时间等指标明显优于B组和A组,而B组患者的各项指标又优于A组(P〈0.05)。结论应用血液灌流联合环磷酰胺治疗可以减轻百草枯对患者各个器官的损害,降低患者的死亡率。 Objective To investigate the effect of hemoperfusion combined with cyclophosphamide on the treatment of acute paraquat poisoning. Methods The data of acute paraquat poisoning cases from July 2007 to February 2010 in our Emergency Department were retrospectively reviewed. All the patients were divided into three groups. Patients in A group were treated with routine therapy. Patients in B group were treated with routine therapy and hemoperfusion. Patients in C group were treated with cyclophosphamide on the basis of B group. Transamiuase,serum creatinine,myocardial enzymogram and mortality were compared. Data of three groups were analyzed by using ANOVA or rank sum test, pariwise comparisons were analyzed by using LSD test, and the rate were compared with ehi - square test. Results A total of 104 patients were chosen for the study. There was no significant difference in age, genders and poisoning dose among the three groups (P 〉 0. 05 ). The function of liver, renal, heart and death rate, survival time were best in the C group than those in two other groups, and the parameters were worst in A group (p 〈 0.05 ). Conclusion Hemoperfusion combined with cyclophosphamide can lessen the organ impairment of patients who suffered from acute paraquat poisoning, and reduce the death rate.
