摘要
目的评价CD44v6、survivin和β-Catenin在细支气管肺泡癌(BAC)的表达及临床意义。方法采用免疫组织化学ElivisionTM两步法检测74例细支气管肺泡癌患者蜡块标本的中CD44v6、survivin和β-Catenin的表达状况,行生存分析研究各因素对BAC预后的预测价值。结果 CD44v6、survivin在Ⅲ期的阳性率明显高于Ⅰ～Ⅱ期患者,有淋巴结转移组的阳性率明显高于无淋巴结转移组(P<0.05)。β-Catenin在低分化组的阳性率明显高于高～中分化组的阳性率(P<0.05)。CD44v6和survivin表达阳性的BAC患者总体生存率低于阴性者(P<0.05)。CD44v6与survivin的表达呈正相关(r=0.257,P<0.05)。单因素和Cox回归多因素分析均显示CD44v6和survivin的表达、TNM分期、N分期和肿瘤直径均为BAC独立的预后影响因素;分层分析显示CD44v6阳性的有淋巴结转移组BAC患者生存期低于阴性者;CD44v6阳性的Ⅲ期BAC患者生存期低于阴性者(P<0.05)。结论 CD44v6与survivin的表达呈正相关,CD44v6和survivin的表达、临床分期、肿瘤直径和N分期都是BAC预后独立的影响因素。
Objective To evaluate the expression and clinical significance of CD44v6,survivin and β-Catenin in BAC.Methods Clinic data of 74 inpatient cases from 1995-2002,were retrospectively reviewed.Expression of CD44v6,survivin and β-Catenin of surgically resected BAC specimens was studied by immunohistochemistry.The value of the three factors in identification of the prognosis of BAC was examined by survival analysis.Results The positive rate of CD44v6 and survivin were all significantly higher in stage Ⅲ and patients with lymphaden metastasis than that in stageⅠ～Ⅱ and without lymphaden metastasis（P0.05）.The positive rate ofβ-Catenin was significantly higher in low histological differentiation than that in high-middle histological differentiation（P0.05）.There was a statistically significant positive correlation between expression of CD44v6 and survivin（r=0.257,P0.05）.Strata analysis suggested that the survival time of BAC with CD44v6 positive was lower than that with CD44v6 negative in nonmucinous types,and the survival time of BAC with CD44v6 positive was lower than that with CD44v6 negative in stage Ⅲ（P0.05）.Cox Regression suggested that CD44v6,survivin,TNM stage,N stage and tumor diameter were independent factors affecting the prognosis of bronchioloalveolar carcinoma.Conclusion The expression of CD44v6,survivin and β-Catenin has a higher specificity,and there is a statistically significant positive correlation.Correlation between expression of CD44v6 and survivin.CD44v6 and survivin,TNM stage,tumor diameter and N stage are all independent factors affecting the prognosis of BAC.
