156例起搏器更换术的原因及经验分析 被引量:1

摘要 目的了解更换起搏器的原因与方法。方法回顾性分析1995年1月~2009年12月共15年间156例更换起搏器的指征和起搏器电极阈值的变化。结果 156例更换起搏器患者,累积更换177例次,因起搏器质保到期129例次,占72.9%;电池耗竭38例次例,占21.5%,其中提前耗竭者6例;因起搏器并发症更换者10例,占5.6%,包括起搏器过敏1例,电极断裂和脱位6例,感染者3例。第1次更换时间平均(94.8±21.9)月,保留电极者79.9%,电极电压阈值从(0.80±0.26)升高到(1.53±0.73);第2次更换平均(85.7±22.8)月,保留电极者占27.8%;更换起搏器类型者11例,占6.2%。结论更换起搏器的主要原因是质保到期,电池耗竭为次要原因,起搏电极平均可应用180个月(15年)左右,更换前仔细评估起搏器及电极的阈值情况,可望减少更换次数。 Aim To investigate the reasons and methods of pacemaker replacement.Method The data of 156 cases with pacemaker replacement were analysed retrospectively from Jan.1995 to Dec.2009.Results There were 177 cumulative times in 156 cases of pacemaker replacement.Of the reason in pacemaker replacement,life-span to maturity were 129 cases,accounting for 72.9%,the battery depletion 38 cases,accounting for 21.5%,the other 10 cases with complications of pacemaker accounting for 5.6%.The average time of the first pacemaker replacement was(94.8±21.9) months.The rate of pacemaker lead retention was 79.9%.The thresholds of pacemaker lead were elevated from(0.80±0.26) to(1.53±0.73),nearly 2 times.The average time of the second pacemaker replacement was(85.7±22.8) months.The rate of pacemaker lead retention was only 27.8%.Pacemaker type were changed in 11 cases,about 6.2% of 177 cumulative times.Conclusions The main reason of pacemaker replacement is life-span to maturity,and battery depletion is the second cause.Pacemaker lead can be used for an average of 180 months(15 years) or so.The number of pacemaker replacement were expected to reduce if the pacemaker and pacemaker lead threshold are carefully evaluated before replacement.
