摘要
目的探讨高血压患者左心室心肌应变特征。方法 40例原发性高血压患者分为2组,其中左心室肥厚(LVH)组15例,非左心室肥厚(NLVH)组25例,另取30例健康志愿者作为对照组,取心尖四腔切面、心尖两腔切面、心尖左心室长轴切面的心肌应变率曲线,测量各室壁收缩期峰值应变率(SRs)、舒张早期峰值应变率(SRe)、心房收缩期的峰值应变率(SRa)。结果 LVH组后壁、下壁、前壁SRs较对照组降低(P<0.05);LVH组及NLVH组各室壁SRe均较对照组降低(P<0.05),但LVH组与NLVH组之间比较差别无统计学意义(P>0.05);LVH组与NLVH组除前间隔外其余室壁SRa与对照组比较差别无统计学意义(P>0.05)。结论应变率显像技术能够客观、准确地评价原发性高血压患者的心肌运动功能。
Objective To explore the characteristics of strain rate of left ventricle in patients with essential hypertension. Methods Forty patients with essential hypertensive were divided into left ventricular hypertrophy( LVH) group( n = 15) ,and non-left ventricular hypertrophy( NLVH) gorup ( n = 25) ,and thirty healthy person were chosed as control group. Strain rate in systolic period( SRs) ,strain rate in early diastolic period( SRe) and strain rate in peak atrial systolic period( SRa) were measured along myocardial strain rate curve of left ventricular long axis views of cardiac apex,cardiac apex four chamber and cardiac apex two chamber. Results SRs of posterior wall ( PW) ,inferior wall ( IW) and anterior wall ( AW) in LVH group were significant lower than those in the control group ( P〈 0. 05) ; All wall ( SRe) in LVH and NLVH group were significant lower than those in the control group ( P〈 0. 05) ; But no significant difference was found between LVH and NLVH group. Except for anterior interventricular septum,SRa of other walls in LVH and NLVH group showed no significance difference compared with the control group. Conclusion The myocardial movement function in essential hypertension patients can be objectively and accurately evaluated by strain rate imaging.
