摘要
目的:探讨妊娠高血压疾病患者外周血ACE水平及其与眼底病变的关系。方法:采用ELISA法检测40例住院HDCP患者(妊娠期高血压、子痫前期轻度、子痫前期重度组)产前和产后血清ACE水平,20例正常妊娠待产孕妇作对照组,分析各组ACE水平及其与眼底病变的关系。结果:不同病情HDCP患者之间产前、产后ACE水平差异有统计学意义(P<0.01),并随着病情加重而升高;各组产前、产后差异有统计学意义(P<0.01),且产后水平较产前下降。不同程度眼底病变之间产前、产后ACE水平差异有统计学意义(P<0.01),且随眼底病变加重呈上升趋势;各组产后水平较产前下降,但只有Ⅲ期眼底病变组有统计学意义(P<0.01)。产前、产后ACE水平与妊娠高血压疾病程度和眼底病变之间有相关性(P<0.01),产前、产后眼底病变与妊娠高血压疾病程度有相关性(P<0.01)。结论:HDCP患者血清ACE水平可能在HDCP眼底视网膜病变发生、发展过程中发挥重要作用,是反映HDCP病情和眼底视网膜病变程度的指标之一。
Objective：To explore the relationship between ACE level in peripheral blood and fundus diseases in patients with hypertensive disorder complicating pregnancy（HDCP）.Methods：The serum levels of ACE before and after delivery in 40 cases with HDCP（gestational hypertension,mild preeclampsia,severe preeclampsia） were detected by ELISA,20 normal prepartal women were selected as control group,the relationship between ACE level in peripheral blood and fundus diseases was analyzed.Results：There was significant difference in ACE levels before and after delivery among HDCP patients with different states of illness（P〈0.01）,and ACE levels increased with development of diseases;there was significant difference in ACE levels before and after delivery（P〈0.01）,ACE level after delivery was lower than that before delivery.There was significant difference in ACE levels before and after delivery among patients with different states of fundus diseases（P〈0.01）,and ACE levels increased with development of diseases;ACE level after delivery was lower than that before delivery,but there was significant difference in stage Ⅲ fundus diseases group（P〈0.01）;there was correlation between ACE levels before and after delivery and degree of HDCP,fundus diseases（P〈0.01）,and there was correlation between fundus diseases before and after delivery and degree of HDCP（P〈0.01）.Conclusion：The serum ACE level in HDCP patients may play an important role in occurrence,development of fundus retinopathy,which is a good index in reflecting disease condition of HDCP and degree of fundus retinopathy.
