摘要
目的评价308nm准分子激光治疗慢性湿疹的疗效及安全性。方法慢性湿疹患者28例,以308nm准分子激光照射皮损,每周治疗2次,连续治疗15次。治疗后随访3个月,评价疗效和安全性。结果 (1)5、10、15次治疗后,有效率分别为42.9%(12/28)、64.3%(18/28)、85.7%(24/28)。(2)5、10、15次治疗后,湿疹面积和严重指数(EASI)评分分别为1.49±1.30、0.88±1.08和0.48±0.70,与治疗前比较差异均有显著性意义(P<0.05)。(3)15次治疗后,红斑鳞屑型和斑块型慢性湿疹EASI评分改善率在分别为(99.93±0.26)%和(70.84±16.09)%,红斑鳞屑型患者EASI评分改善率均明显高于斑块型(P<0.01)。(4)28例患者平均照射次数为11.5次,平均累积照射剂量为(8.48±5.95)J/cm2。其中红斑鳞屑型和斑块型分别是(4.75±2.20)和(13.74±4.99)J/cm2,斑块型患者的平均累积照射剂量均明显高于红斑鳞屑型(P<0.05)。结论 308nm准分子激光治疗慢性湿疹安全、有效。红斑鳞屑型慢性湿疹患者的疗效明显高于斑块型慢性湿疹。
Objective To investigate the clinical efficacy and safety of 308 nm excimer laser on the chronic eczema.Methods Twenty-eight patients with chronic eczema were treated by a 308 nm excimer laser which was performed two times per week,and continued totally for 15 times.Follow-ups were scheduled on 3 months after the treatment.The therapeutic effect and safety were evaluated.Results（1）After 5,10 and 15 times of treatment,the effective rate of 28 patients with chronic eczema was 42.9%,64.3% and 85.7%,respectively.（2）After 5,10 and 15 times of treatment,the eczema area and severity index （EASI） scores of 28 patients with chronic eczema were（1.49±1.30）,（0.88±1.08） and （0.48±0.70）,respectively.The scores after treatment were significantly lower than before treatment（P0.05）.（3）After 15 times of treatment,the improved rate of EASI in the erythema type and the plaque type of chronic eczema was（99.93±0.26）% and （70.84±16.09）%,respectively,the former was higher than the latter（P0.01）.（4）After 15 times of treatment,the average cumulative dose of excimer laser in the erythema type and plaque type of chronic eczema was （4.75±2.20） J/cm^2 and （13.74±4.99） J/cm^2,respectively,the former was also higher than the latter（P0.05）.ConclusionsThe clinical data demonstrate that the treatment of 308 nm excimer laser is safe and effective.The efficacy on the erythema type of chronic eczema is better than on the plaque type.
