摘要
目的研究慢性心力衰竭(CHF)相关的各种因素与CHF患者1年内死亡的关系。方法连续收集了CHF住院患者179例,随访记录人口学资料、心脏功能、住院原因、治疗情况、治疗依从性、TNT及出院一年内的死亡情况,以一年内的死亡为研究终点,通过多因素Logistic回归分析,探讨以上各因素与死亡的关系。结果 TnT阳性(OR=8.694,95%的CI为2.599-29.081,P<0.001)、院外不规律服药(OR=7.019,95%的CI为2.307～21.349,P<0.001)、COPD(OR=4.078,95%的CI为1.046～15.898,P<0.001)是死亡的危险因素。结论院外不规律服药、TnT阳性、合并COPD是CHF患者一年内死亡的主要危险因素。
Objective To investigated the effects of factors mainly related to the death in patients with CHF discharged within 1 year.Methods We collected sequentially 179 pts with CHF.We recorded data about the durations in hospital including demographic factors,heart functions,reason for hospitalization,treatment,compliance to therapy,troponin T（TnT） levels and death within 1 year after discharge.The death were considered as primary end point events.We analysed the data by multivariate logistic regression to investigate the relationships between those factors and the end point events.Results By multivariate logistic regression analysis,we found that three variables including TnT levels（≥0.01 ng/mL）（OR 8.694,95% CI 2.6-29.1,P0.001）,poor medications compliance（OR 7.019,95% CI 2.3-21.3,P0.001） and with COPD（OR 4.078,95% CI 1.0-15.9,P0.001）,were identified as significant independent predictors for death.Conclusion TnT levels,poor medications compliance,with COPD were identified as significant independent predictors for death.
