摘要
目的 :通过测定C—反应蛋白 (CRP)在慢性心力衰竭 (CHF)患者的血浓度 ,试图里了解CRP在CHF发生发展过程中的作用。方法 :选择心功能Ⅱ~Ⅳ (NYHA)CHF患者 77例 ,年龄 <6 0岁 30例 ,≥ 6 0岁 4 7例 ;伴有低钠血症者 (<1 35mmol/L) 34例 ,不伴低钠血症者 4 3例。 36例正常志愿者 ,年龄5 6 4± 1 6 0 (36~ 82 )岁。采用免疫比浊法测定血清CRP ,放免法测定血浆BNP。结果CHF患者的血清CRP、血浆BNP含量显著高于对照组 (P <0 0 0 1 ) ,且含量随心功能恶化而明显升高 ,Ⅱ~Ⅳ级组间差异有显著性意义 (P <0 0 5 ) ,年龄组间无显著性差异。CRP与LVEF、血钠呈负相关 (r =- 0 1 97,P <0 0 5 ;r =- 0 2 95 ,P <0 0 1 )。结论 :随NYHA心功能分级级别增高 ,血清CRP水平明显升高 ,CRP可能是心衰恶化的标志物之一 ,但心衰CRP增高与年龄分组关系不密切。
Objective this investigation aims at determination the concentration of CRP in serum in CHF and clarification the role of CRP in the development of CHF. Method 77 patiens with CHF and without infection or inflammatory disease was enrolled, whose cardiac function is atⅡ-Ⅳ(NYHA), ages at 33 - 85 (average 63.1 ± 12.6) years. 36 health volunteers, ages at 36 - 82 (average 56.4 ± 16.0), 34 patients with Hyponatremia and 43 patients without Hyponatremia in CHF. We determine the concentration of CRP in serum by immunoturbidmetry and the concentration of BNP in plasm by radioimmunoassay in these patiens. Result the concentration of CRP in serum as well as the concentration of BNP in plasm in CHF is significantly higher than control group ( P 〈 0.001 ), and the higher rank of NYHA cardiac function classification, the higher levels of CRP and BNP, there is significant difference among the groups of Ⅱ-Ⅳ (NYHA), there was significant negative correlation between the plasma levels of CRP, BNP and senan sodium eoncemtration ( r = - 0. 197, P 〈 0.05; r = - 0.295, P 〈 0.01). Conclusion the level of CRP in serum increases significantly with the NYHA cardiac function classification elevating. Therefore, CRP maybe one of the signs of the deterioration of cardiac function.
