摘要
目的总结消化性溃疡穿孔合并胆囊结石、胆囊炎的腹腔镜同期手术诊治经验。方法对该院2005年3月～2009年5月收治的急性消化性溃疡穿孔合并胆囊结石、胆囊炎患者17例,均在气管插管、静脉麻醉下一期行腹腔镜下消化性溃疡穿孔修补术及胆囊切除术。结果 15例胃溃疡穿孔及2例十二指肠溃疡穿孔患者均手术治愈,无死亡病例,无严重并发症发生,手术效果良好,患者术后恢复满意,营养状况良好。结论腹腔镜下消化性溃疡穿孔修补术联合胆囊切除术安全可行,在不增加患者创伤的情况下能同时去除病灶,有利于术后患者营养状况的改善,能很快地促进患者术后康复,疗效良好,值得临床推广应用。
【Objective】To summarized the clinical experience in laparoscopic repair of peptic ulcer perforation combined laparoscopic cholecystectomy of gallbladder caculi. 【Methods】17 patients with peptic ulcer perforation and gallbladder caculi were performed by laparoscopic repair and cholecystectomy under intravenous-inhalation general anesthesia after a histological biopsy since March 2005 to May 2009. 【Results】All 15 cases with gastric perforation and 2 patients with duodenum perforation were successfully operated and recovered well. There were no serious complications occurred and no operative mortality in this series,this operation was effective,and patients were in good nutritional status.【Conclusions】Laparoscopic repair combined laparoscopic cholecystectomy in peptic ulcer perforation with gallbladder caculi is an effective and safe treatment,it can remove the both niduses in miniinvasive method. And it can improve the patient＇s nutritional status,the patients can recover quickly.
