脑钠肽及高敏C反应蛋白与脑出血患者血肿周围水肿的相关性 被引量:5

Relationship between the plasma levels of brain natriuretic peptide and high sensitive C-reactive protein and brain edema around hematoma in patients with intracerebral hemorrhage

摘要 目的探讨血浆脑钠肽及高敏C反应蛋白(HsCRP)水平与脑出血患者血肿周围水肿之间的关系。方法选取基底节脑出血患者65例,分别在当天、3d检测血浆脑钠肽和HsCRP水平、头颅CT扫描、NIHSS量表评分,并测定62例与其性别、年龄相匹配健康体检者脑钠肽和HsCRP作为对照。结果脑出血组血浆脑钠肽、HsCRP明显高于对照组(P<0.01);脑钠肽水平高于100pg/mL患者血浆HsCRP显著高于脑钠肽小于100pg/mL患者,且前者血肿周围水肿更显著,神经功能缺损评分亦明显高于后者(P<0.01)。结论血浆脑钠肽及HsCRP水平与脑出血以及周围脑水肿、神经功能缺损程度相关,提示脑钠肽、HsCRP对脑出血后脑水肿及继发性颅脑损伤可能有预测价值。 [Objective] To investigate the association between plasma levels of natriuretic peptide (BNP) and high sensitive C-reactive protein (HsCRP) and brain edema around hematoma in patients with intraeerebral hemorrhage. [Methods ] 55 consecutive patients with intracerebral hemorrhage were determined of BNP and HsCRP and stroke severity was measured by means of the NIHSS on 1 d and 3 d respectively. Brain CT was examined at admittance and on 3d. The levels of BNP and HsCRP were also measured in 62 healthy control volunteers. [Results] The plasma levels of BNP and HsCRP were significantly higher in patients than in control group (P 〈0.01). Compared with those patients who had normal BNP levels (〈100 pg/mL), patients who had elevated BNP levels (≥ 100 pg/mL) had significantly higher plasma levels of hs-CRP and NT-proBNP, larger volume of brain edema around hematoma and remarkably higher scales by NIHSS (P 〈0.01). [Conclusions] The plasma levels of BNP and HsCRP are positively correlated with brain edema around hematoma and stroke severity. These findings indicate that increase of BNP and HsCRP levels may be used to anticipate brain edema and secondary injury after intracercbral hemorrhage.
