摘要
目的调查分析东北地区2008年临床分离细菌的菌谱构成以及对抗菌药物的耐药性。方法采用K-B纸片扩散法或仪器法对2008年东北地区12家医院临床分离细菌进行药敏试验,依据CLSI2009年标准,采用WHONET5.5软件进行统计分析。结果东北地区12家医院共分离细菌23738株,其中革兰阴性菌17075株,占71.9%,革兰阳性菌共6663株,占28.1%。大肠埃希菌、肺炎克雷伯菌和奇异变形菌产超广谱β-内酰胺酶(ESBLs)的产酶率分别为58.78%、43.9%和37.8%。肠杆菌科细菌对亚胺培南仍保持较高的敏感性(>98%)。大肠埃希菌对左氧氟沙星的耐药率为69%。铜绿假单胞菌、鲍曼不动杆菌和洋葱伯克霍尔德菌对美罗培南的耐药率分别为27.2%、41.5%和26.1%。MRSA和MRSCN检出率分别为64.1%和73.9%;未检测到万古霉素耐药的葡萄球菌;屎肠球菌和粪肠球菌对万古霉素的耐药率为1.8%、2.5%;对替考拉宁的耐药率为1.2%,2.6%。结论细菌的耐药问题仍很严重,对细菌耐药性变化应引起足够重视,加强耐药监测工作以及合理使用抗菌药物对于降低细菌的耐药性,控制泛耐药细菌的传播和爆发流行有重要意义。
Objective To investigate bacterial profile and the antibiotic resistance in clinical isolates in northeast China in 2008. Methods Clinical bacterial isolates were collected from 12 hospitals in northoast China and antibiotic susceptibility test was carried out by means of K-B disk diffusion or automatic machine. The data was analyzed by WHONET5.5 software according to CLSI 2009 standards. Results 23,738 strains were isolated from 12 hospitals in northeast China, 17,075 of which were Gram-negative bacteria, account for 71.9%, 6,663 of them were Gram-positive, account for 28.1%. The rate of extend spectrum-β-1actamases (ESBLs) production ofEscherichia coli, Klebsiella pneumoniae and Proteus mirabilis were 58.78%, 43.9% and 37.8%. Enterobacteriaceae maintained the higher susceptibility to imipenem (〉98%). Resistant ratio of E.coli to levefloxacin was 69%. lmipenem resistant rates ofPseudomonsa aeruginosa, Acinetobacter baumannii and Burkholderia cepacie were 27.2%, 41.5% and 26.1%. The rates of MRSA and MRSCN were 64.1% and 73.9%. Staphylococcus spp. resistant to vancomycin was not found in this study. Rates of Enterococcusfaecium and E. faecalis resistant to vancomycin were 1.8% and 2.5%, to teicoplanin were 1.2% and 2.6%. Conclusion Problem of drug resistance of bacteria was serious. Much attention should be paid to variety of drug resistance. It is significant and important to enforce drug resistance monitoring, optimum selection of antibiotic, and to control the dissemination and prevalent outbreak ofpanresistant bacteria.
