摘要
目的:研究白血病抑制因子(Leukemia inhibitory factor,LIF)在子宫内膜异位症(简称内异症,Emdometrio-sis,EMS)患者种植窗期子宫内膜排卵期卵泡液中的表达情况,探讨其与EMS患者IVF-ET(In vitro fertilization-embryotrans-fer)妊娠率下降的关系。方法:利用23例不孕合并EMS患者签订知情同意书的情况下IVF前一周期种植窗期取子宫内膜,取卵日收集废弃的卵泡液。采用免疫组化SABC法检测内膜LIF,双抗体酶联免疫吸附法(ELISA)测定取卵日卵泡液中LIF含量,对照组是47例非EMS患者。分析LIF的表达情况以及与胚胎发育、妊娠的关系。结果:非EMS患者与EMS患者获卵数(13.86±8.25)和(9.43±7.12)、受精率(76.05%和60.59%)、可用胚率(54.28%和34.47%)、胚胎种植率26.53%(26/98)和8.51%(4/27)、临床妊娠率51.06%(24/47)和17.39%(4/23),差异均有统计学意义(P<0.05);LIF在取卵期卵泡液中分别为(25.10±10.16)pmol/L和(14.68±5.54)pmol/L,差异有统计学意义(P<0.05)。LIF在种植窗期子宫内膜表达情况,EMS组显著低于非EMS组(P<0.05)。结论:EMS患者种植窗期的子宫内膜、取卵日卵泡液中LIF表达均下降,可能与EMS导致不孕有相关性。
Objective： To study the expression status of leukemia inhibitory factor （ LIF） in endometrium of implantation window phase and follicular fluid of ovulatory phase in patients with endometriosis,explore the relationship with decrease of pregnancy rate in patients receiving in vitro fertilization embryo transfer （ IVF ET） . Methods： The samples of endometrium from 23 patients with endometriosis were obtained before IVF,and obsolete follicular fluid on the day of ovum pick up was collected; the expression of LIF in endometrium was detected by immunohistochemical SABC method,and the content of LIF in follicular fluid on the day of ovum pick up was detected by double antibody enzyme linked immunosorbent assay,47 women without endometriosis were selected as control group; the expression of LIF and its relationship with embryo development and pregnancy were analyzed. Results： The numbers of retrieved oocytes,fertilization rates,available embryos rates,implantation rates,clinical pregnancy rates in control group and endometriosis group were （ 13. 86 ± 8. 25） and （ 9. 43 ± 7. 12） ,76. 05% and 60. 59%,54. 28% and 34. 47%,26. 53% （ 26/98） and 8. 51% （ 4/27） ,51. 06% （ 24/47） and 17. 39% （ 4/23） ,respectively,there was significant difference between the two groups （ P 0. 05） . The levels of LIF in follicular fluid of ovulatory phase were （ 25. 10 ± 10. 16） pmol/L and （ 14. 68 ± 5. 54） pmol/L,respectively,there was significant difference （ P 0. 05） ; the level of LIF in endometrium of implantation window phase in endometriosis group was significantly lower than that in control group （ P 0. 05） . Conclusion： The decrease of LIF expression levels in endometrium of implantation window phase and follicular fluid of ovulatory phase may be related to infertility induced by endometriosis.
