摘要 目的探讨尿微量蛋白检测对原发性高血压病患者早期肾损害诊断的临床价值。方法选取原发性高血压病患者86例用竞争免疫分析法检测尿α1-微球蛋白(Uα1-MG)、尿β2-微球蛋白(Uβ2-MG)、尿白蛋白(UALB)、尿免疫球蛋白(UIGG),并与40例正常健康者作比较。结果与正常对照组比较,原发性高血压病患者的UIGG增高显著(p〈0.05),Uα1-MG、Uβ2-MG、UALB增高非常显著(P〈0.01);与高血压1级患者比较,高血压2—3级患者的Uα1-MG、Uβ2-MG、UALB增高非常显著(P〈0.01);与高血压1级患者比较,高血压2~3级患者Uα1-MG、Uβ2-MG、UALB检测的阳性率显著升高(P〈0.05)。结论尿微量蛋白检测对原发性高血压早期肾损害的诊断有帮助,能反映原发性高血压患者早期肾功能损害的部位和程度,其中以Uα1-MG、Uβ2-MG、UALB最敏感,定期检测对发现原发性高血压早期肾损害有重要的临床价值。 Objective To investigate the clinical value of urinary microalbumin detection for the diagnosis of early renal injury in patients with primary hypertension. Methyl8 Urinary levels of al- micraglobulin (Uα1-MG), β2-microglobulin (Uβ2-MG), albumin (UALB), and immune globulin (UIGG) were measured by competition immunoassay in 86 patients and 40 healthy subjects. Results As compared with the healthy subjects, UIGG level increased significantly (P〈0.05) and levels of Uα1-MG, Uβ2-MG, and UALB elevated markedly in patients with primary hypertension (P〈0.01). As compared with patients with grade 1 hypertension, levels and detection rates of Uα1-MG, Uβ2-MG, and UALB increased obviously in patients with grades 2-3 hypertension (P〈0.01 and P〈0.05). Conclusions Urinary microalbumin is helpful in diagnosing early renal injury in primary hypertension, reflecting the site and degree of renal injury. α1-mieraglobulin, β2-microglobulin, and albumin are the most sensitive indicators. Regular detection of urinary microalbumin has important clinical values.
