摘要
目的研究特发性血小板减少性紫癜(ITP)患儿经治疗血小板数量正常后的血小板功能、巨核细胞数量及T淋巴细胞亚群的变化。方法通过玻璃柱法测定血小板粘附功能;用自发性血小板聚集试验测定血小板聚集功能;血小板第3因子有效性测定血小板凝血活性;T淋巴细胞亚群的测定是采取间接免疫荧光法。结果(1)骨髓内巨核细胞总数、幼巨核数、过渡型及裸核数在治疗后均明显减少,分别由治疗前的150±85降至68±25;50±35降至5±1;80±44降至17±12;66±40降至10±5,而成熟型由7±5升至38±15(P<0.001)。(2)血小板粘附率在血小板计数正常后为(25.5±10.1)%,而正常对照组为(56.6±11.1)%(P<0.001)。(3)76.2%的ITP患儿出现T4/T8比值倒置,T4明显低于正常对照组。结论ITP患儿血小板数量正常后的一段时间内仍存在血小板粘附功能下降和T淋巴细胞亚群异常。
Objective To study the changes in bone marrow megalokaryocyte numbers, platelet function and T lymphocyte subsets in children with idiopathic thrombocytopenic purpura (ITP) after treatment and normalization of platelet count. Methods Platelet adhesion was determined by using glass bead column methods. Platelet aggregation was determined by using a spontaneous platelet aggregation test. Platelet coagulation activity was detected by using a platelet facter 3 activity test (PF3aT). And T lymphocyte subsets were detected with an indirect immunofluorescence method. Results ①After treatment the numbers of megalokaryocyte, megalokaryoblast, early megalokarycyte and naked megalokaryocyte in bone marrow were significantly decreased from 150±85 to 68±25, 50±35 to 5±1, 80±44 to 17±12 and 66±40 to 10±5, respectively. While mature megalokaryocytes were increased from 7±5 to 38±15 ( P <0.001). ②Platelet adhesion rate was significantly decreased in patients with ITP (25.5±10.1)% compared to control group (56.6±11.1)% ( P <0.001). ③The ratio of T 4/T 8 showed abnormal in 76.2% of patients with ITP. T 4 was significantly lower in patients with ITP than that in control group. Conclusion The abnormalities of platelet adhesion rate and T lymphocyte subsets will present for a while even though the platelet count has returned to normal level after treatment in patients with ITP.
