摘要
目的:探讨血清高敏C-反应蛋白(hs-CRP)和脉搏波传导速度(PWV)对冠心病诊断及冠脉病变严重程度的预测价值。方法:入选114例冠脉造影阳性的心绞痛患者为冠心病组,50例冠脉造影阴性者为正常对照组。根据心绞痛类型冠心病组进一步分为稳定型心绞痛组(SAP组,50例)和不稳定型心绞痛组(UAP组,64例);又根据冠脉造影结果分为单支病变组(24例)、双支病变组(26例)和三支病变组(64例),并计算冠脉病变积分。测定各组患者hs-CRP和PWV,并将其分别与冠脉病变支数及积分进行相关性分析。结果:冠心病组的hs-CRP和PWV均显著高于正常对照组(P<0.01),UAP组的hs-CRP和PWV均显著高于SAP组[(6.86±1.36)mg/L:(3.75±1.03)mg/L,(11.59±1.32)m/s:(9.60±1.75)m/s,P均<0.05];双支病变组的hs-CRP和PWV较单支病变组明显升高[(5.32±1.17)mg/L:(3.05±0.95)mg/L,(10.51±1.23)m/s:(9.84±1.62)m/s,P均<0.05],三支病变组的hs-CRP和PWV较双支病变组明显升高[(7.21±1.13)mg/L:(5.32±1.17)mg/L,(11.30±2.65)m/s:(10.51±1.23)m/s,P均<0.05]。hs-CRP水平与冠脉病变支数、冠脉病变积分呈正相关(r=0.56,0.32,P均<0.01);PWV与冠脉病变支数、病变积分呈正相关(r=0.29,0.37,P均<0.01)。结论:联合检测血清hs-CRP和PWV对诊断冠心病、评价冠脉病变严重程度有一定价值。
Objective: To investigate the relationship among high sensitivity-C reactive protein (hs-CRP), pulse wave velocity (PWV) and coronary heart disease. Methods: A total of 164 patients who received coronary angiography (CAG) were allocated into two groups: normal control group (n= 50) and angina pectoris group (n= 114). In the angina pectoris group, 50 patients were stable angina pectoris (SAP) and 64 patients were unstable angina pectoris (UAP) ; 24 patients with single vessel coronary disease, 26 patients with two-vessel coronary disease, 64 patients with three-vessel coronary disease. The lesion score of CAG, serum hs-CRP and PWV were measured in all groups. Results: The serum hs-CRP and PWV levels were significantly higher in angina pectoris group than those in normal control group (P〈0.01). Among angina pectoris group, the serum hs-CRP and PWV levels were significantly higher in UAP group than those in SAP group [ (6.86±1.36) mg/L vs. (3.75±1.03) mg/L, (11.59±1.32) m/s vs. (9.60±1.75) m/s, P〈0.05 all]. The hs-CRP and PWV levels in with two vessel coronary disease group where higher than those of single vessel coronary disease group [ (5.32±1.17) mg/L vs. (3.05±0.95) mg/L, (10.51±1.23) m/s vs. (9.84±1.62) m/s, P〈0.05 all]. The hs-CRP and PWV levels in with three vessel coronary disease group increased compared with those of two vessel coronary disease group [ (7. 21± 1.13) mg/L vs. (5.32±1.17) mg/L, (11.30±2. 65) m/s vs. (10.51±1. 23) m/s, P〈0.05 all]. Pearson relation analysis showed that serum hs-CRP was positively related with the number of coronary artery disease and lesion score (r=0.56 and 0. 32 respectively, P〈0.01). PWV was positively related with the number of coronary artery disease and esion score (r= 0. 29, 0. 37, P〈0. 05, P〈0. 01 respectively). Conclusion: Combined detect Serum bs--CRP and PWV can be used as an assistant indexes for diagnosing and classification of angina pectoris.
