摘要
【目的】对慢性阻塞性肺疾病急性加重期（AECOPD）合并肺结核（PTB）的临床特点进行回顾性分析，探讨PTB对AECOPD的影响。【方法]2007年3月到2010年3月本院呼吸内科住院的AECOPD合并PTB患者41例，其中陈旧性PTB（A组）23例，活动性PTB（B组）18例，另选则同期AECOPD患者无结核（C组）21例，分析比较上述患者各临床相关指标以及住院费用、住院时间等。【结果】与C组相比，B组感染指标淋巴细胞（L％）、总淋巴细胞计数（TLC）、血沉（ESR）、C反应蛋白（CRP）较高，营养学指标血红蛋白（Hb）、白蛋白（ALB）和体重指数（BMI）较低，差异有显著性（P〈0．05）；而13组感染指标N％、L％、ESR、CRP高于A组，营养学指标Hb、ALB低于A组，差异有显著性（P〈0．05）；AECOPD合并PTB患者住院费用高、住院时间长。【结论]PTB对AECOPD的临床进程有影响，主要表现在AECOPD合并PTB病情重以及营养状态较差、住院费用增高、住院时间较长。
[Objective]To retrospectively analyze the clinical features of acute exacerbation of chronic obstructive pulmonary disease（AECOPD） complicated with pulmonary tuberculosis （PTB）, and to explore the effect of PTB on AECOPD. [Methodsl Forty one cases of AECOPI） complicated with PTB admitted in our hospital from March 2007 to March 2010 were divided into two groups. There were 23 cases of old PTB in group A and 18 cases of active PTB in group B. Other 21 cases of AECOPD without PTB were selected as group C. Clinical relevant indicators, hospitalization costs and length of stay were analyzed and compared among these groups. [Results]Compared with group （2, the infection indicators such aslymphoeytes（L%）, total lymphocyte count（TLC）, erythrocyte sedimentation rate （ESR） and c reactive protein （CRP） in group B were higher, and the nutrition indicators such as hemoglobin（Hb）, albumin（AI.B） and body mass index（BMI） were lower, and there were significant differences between group C and group B（ P 〈0.05）. The infection indicators of group B were higher and the nutrition indicators were lower than those of group A, and there were significant differences between group B and group A（ P 〈0.05）. Group B had higher hospitalization cost and longer hospital stay. [Conclusion]PTB has the effect on the clinical process of AECOPD. AECOPD complicated with PTB has poorer nutritional status, higher hospitalization cost and longer hospital stay.
