摘要
目的:观察胰岛素外用对糖尿病大鼠深Ⅱ度烧伤创面中缺氧诱导因子-1α(Hypoxia inducible factor-1α,HIF-1α)和血管内皮生长因子(Vascular endothelial growth factor,VEGF)表达的影响,探讨胰岛素外用促进糖尿病创面愈合的可能机制。方法:SD大鼠42只,雌雄不限,建立链脲菌素(Streptozotocin,STZ)诱导的糖尿病大鼠深Ⅱ度烧伤模型,完全随机分为胰岛素外用组及空白对照组,即分别用胰岛素及生理盐水敷料每日换药两次。于治疗1、2、3周后,取创面全层组织,通过RT-PCR和免疫组化分别检测创面组织中HIF-1α mRNA的含量的变化和HIF-1α及VEGF的阳性表达情况以及微血管密度。结果:与对照组相比较,胰岛素干预1、2、3周后,创面愈合率明显增大,创面组织中HIF-1α mRNA相对含量和HIF-1α的阳性细胞数明显减少,而VEGF的阳性表达细胞数则明显增多(P<0.05),且微血管密度也显著增高(P<0.05)。结论:胰岛素外用能促进糖尿病大鼠创面的愈合,其机制与下调HIF-1α的水平、上调VEGF的表达水平,进而促进创面血管生长,改善创面的血供有关。
Objective:To study the effect of dressing with insulin on the expression of HIF-1αand VEGF in deepⅡ degree burn wound healing of diabetic rats and explore the possible mechanisms. Methods:42 rats,either male or female,were established models of deepⅡ burn wounds with diabetes,which was induced by streptozotocin (STZ). They were completely randomly divided into two groups,that is the insulin group and the control group. Dressing changes with insulin and saline dressings twice daily,respectively. Full-thickness tissues of the wounds were collected respectively in the 1st,2nd and 3rd weeks after the treatment. The expression of hypoxia inducible factor-1αmRNA (HIF-1αmRNA)was detected with RT-PCR;and the positive staining of HIF-1αand VEGF and microvessel density in the wound healing were determined with IHC. Results:Compared with the control,the healing rate was obviously increased, the expression of HIF-1αmRNA and the positive expression of HIF-1αwere significantly decreased (P〈0.05),while,the expression of VEGF were obviously strengthened (P〈0.05)in the insulin group after 1w,2w,3w. And,the microvessel density was significantly higher in the insulin group than that in the control. Conclusion:Dressing change with insulin can promote the wound healing in diabetic rats through down-regulating the expression of HIF-1α,up-regulating the expression of VEGF in the wound,thereby inducing the growth of blood vessels of wound,and improving the blood supply.
