摘要
目的观察70岁以上老年食管癌三维适形放疗的临床疗效和副反应。方法 45例食管癌患者均采用三维适行放疗,肿瘤剂量50~62 Gy。结果 45例老年食管癌患者放疗后完全缓解（CR）17例（37.8%）,部分缓解（PR）24例（53.3%）,有效率91.1%（41/45）。1,2年生存率分别为68.8%和37.8%。死亡原因中局部复发15例,占53.6%;远处转移7例,占25.0%。结论三维适形放射治疗老年食管癌对缓解症状、提高生存质量起到了积极作用,放射性肺炎的发生与老年人基础疾病有关。
Objective To Observe the clinical efficacy and side effects of three dimensional conformal radiation therapy in treatment of over 70 years-old patients with esophageal carcinoma.Methods Three dimensional conformal radiation therapy was given to 45 patients with esophageal carcinoma and the total tumor dose was 50-62 Gy.Results After the radiation therapy for 45 patients with esophageal carcinoma,the complete remission（CR） was 37.8%（17/45）,partial remission（PR） was 53.3%（24/45） and the effective rate was 91.1%（41/45）.The 1-,2-year survival rates were 68.8% and 37.8% respectively.Among the causes of death,15 cases died of local recurrence,accounting for 53.6% and 7 cases died of distant metastasis,accounting for 25.0%.Conclusion Three dimensional conformal radiation therapy can relieve symptoms and improve survival quality for elderly patients with esophageal carcinoma.The occurrence of radiation pneumonitis is related to basic diseases in elderly patients.
