摘要
目的:探讨卵巢肿瘤组织中nm23-H1的表达和突变与其浸润转移的关系。方法:采用免疫组织化学,反转录多聚酶链反应(RT-PCR)和多聚酶链反应单链构象多态技术(PCRSSCP)方法检测41例卵巢癌,20例卵巢良性肿瘤和21例正常卵巢组织nm23-H1的表达和突变。结果:(1)卵巢癌组织中nm23-H1阳性表达率明显高于良性卵巢肿瘤及正常对照(P<001);(2)卵巢癌灶nm23-H1的阳性表达及扩增明显高于转移灶(P<001);(3)Ⅰ~Ⅱ期患者癌组织中nm23-H1的阳性表达率明显高于Ⅲ~Ⅳ期者(P<001);(4)nm23H的阳性表达和扩增与病理分级及病理分类无显著差别(P>005);(5)nm23H1阳性表达的患者近期疗效比阴性者好(P<005)。(6)卵巢癌组织中nm23-H1的突变率为73%且均发生在晚期患者,而良性肿瘤和正常对照未发现有突变。结论:nm23-H1阳性表达与卵巢癌的浸润转移有关,对它的检测有助于临床上判断预后。
Objectives:To observe the amplification and expression of nm23 H1 gene and investigate their clinical significance in ovarian cancer.Methods:Immunohistochemistry,reverse transcription polymerase chain reaction(RT PCR)and single strand conformation polymorphisms(PCR SSCP)procedure were used to detect the mRNA,protein and mutation of nm23 H2 gene in 41 ovarian cancer,20 benign tumors and 21 normal controls.Results:(1) The positive expression of nm23 in ovarian cancer was significantly higher than that in benign tumors and normal controls( P <0 01).(2)The positive expression rate of nm23 H1 gene in primary focus was significantly higher than that in adjacent sites and metastatic lesions( P <0 01).(3) The positive expression rate of nm23 H1 gene in Ⅰ~Ⅱ stages of patient was more higher than that in Ⅲ~Ⅳ stages of patient( P <0 01).(4) No significant correlation was observed between the positive expression rate of nm23 H1 gene and pathological type or grade in ovarian cancer( P >0 05).(5) The short term curative effects in patient with positive expression of nm23 H1 gene was better than that with low expression( P <0 05).(6)The mutation rate of nm23 gene in ovarian carcinoma was 7 3%,but there is no any mutation in benign tumors and normal controls.Conclusion:The expression of nm23 gene is correlated with the metastasis of ovarian carcinoma and may be of value clinically in detecting the metastasis of tumor and in predicting prognosis.
