摘要
目的:探讨亚低温治疗对重型颅脑损伤患者(Severe traumatic brain injury,STBI)脑脊液中肿瘤坏死因子-α(Tumornecr osis factor alpha,TNF-α)、白介素6(Interleukin-6,IL-6)及白介素8(Interleukin-8,IL-8)的影响及临床意义。方法:选择符合条件的STBI共40例者(格拉斯哥评分(Glasgow Coma Scale score,GCS)≤8分,伤后6h内入院)随机分为亚低温组和常温对照组各20例,采用放射免疫法检测亚低温治疗前后不同时段脑脊液(Cerebrospinal fluid,CSF)中IL-6、IL-8及TNF-α水平的变化。结果:亚低温治疗组STBI血清IL-6、IL-8及TNF-α浓度较常规治疗组明显降低(P<0.05)。结论:亚低温治疗STBI通过细胞因子的水平,减少了患者多器官功能障碍综合征(Multiple organfailure,MODS)的发生,降低了死亡率,改善了临床预后。
Objective:To investigate the effect of moderatehy pothermia on tumor necrosis factor alpha(TNF-α),interleukin-6(IL-6) and intedeukin-8(IL-8)in cerebrospinal fluid(CSF)of patients with severe traumatic brain injury(STBI)and its clinical significance.Methods:Forty patients with severe traumatic brain injury were randomly divided into hypothermic group and control group.Inclusion criteria included Glasgow Coma Scale(GCS)score ≤8 and time of after injury to on admission must be within 6 hours.TNF-α,IL-6,IL-8concentrations of CSF were tested before and after hypothermic therapy by radioimmunoassay.Results:Levels of serum TNF-α,IL-6 and IL-8 were decreased more significantly in the moderate hypothermia group than the control group(P〈0.05).Conclusion:Moderate hypothermia can decrease cytokines levels in patients with severe brain injury and hence reduce incidence of multiple organ dysfunction syndrome,the mortality and improve the clinical prognosis of STBI.
