抗细胞间粘附分子-1抗体对心肌再灌注损伤的保护

摘要 目的研究抗细胞间粘附分子-1抗体对心肌再灌注损伤的保护作用。方法取12只杂种狗,随机分成对照组和实验组,常规建立体外循环,于主动脉根部灌注冷晶体停搏液(实验组中加用抗ICAM-1单抗1.0mg·kg~-1)。在主动脉阻断前和心脏停跳90min再灌注30min时,分别检测心功能,从左心室部活检取心肌组织测丙二醛(MDA),抽取冠状窦血测肌酸磷酸激酶(CPK)。结果与对照组相比,实验组在再灌注后心功能恢复率有明显提高,CPK和MDA下降(P<0.05)。结论在体外循环过程中,抗ICAM-1抗体对心肌再灌注损伤具有保护作用,亦间接表明ICAM-1在心肌再灌注损伤中发挥重要的作用。 ObjectiveIn order to elucidate the effect of monoclonal antibody (MAb) to ICAM -1 on myocardial ischemic reperfusion injury during cardiopulmonary bypass. Methods Twelve mongrel dogs were randomly divided into two groups: control group (n=6) and experimental group (n=6). Cardiopulmonary bypass was conventionally instituted and cold crystalloid cardioplegia(experimental group added MAb to ICAM - 1 1mg. kg^-1) was perfused into the root of aorta. Blood samples from coronary sinus were taken before aortic clamping and 30min after coronary reperfusion for changes of creatine phosphokinase (CPK), specimens of left ventricular tissue were biopsied to analyze the myocardial malonaldehyde (MDA) content, and cardiac function was measured at the corresponding time points. Results In experimental group, compared with control group, the recovery of myocardial dysfunction was ameliorated and MDA, CPK were found reduced (P< 0.05) after myocardial reperfusion. ConclusionOur study indicated that MAb to ICAM - 1 could have protective effect on myocardium after ischemia/reperfusion during cardioplumonary bypass, suggesting that ICAM -1 plays a pivotal role in myocardial ischemia/reperfusion injury.
