摘要
目的:探讨鼻咽癌后程三维适形放射治疗(3DCRT)的疗效。方法:40例初治无远处转移的鼻咽癌患者,采用分两阶段对原发灶及颈部放射治疗。第一阶段用面颈联合野常规放疗DT 38Gy,第二阶段用3DCRT DT30-36Gy,鼻咽癌原发灶总剂量70-74Gy,颈部用切线野常规分割放疗,射野上界保持与主野下界衔接,对颈淋巴结阴性照射DT 50Gy,淋巴结阳性照射DT 66-70Gy。结果:局部肿瘤消退率(有效率)100%,完全缓解率93.85%,1年生存率100%。无严重并发症发生。结论:鼻咽癌常规放射治疗加后程三维适型放射治疗近期疗效满意。
Objective：To observe the short - term efficacy of late - course 3 - dimensional conformal radiation therapy （3DCRT） for nasopharymgeal carcinoma（NPC）, nehtods： For 40 patients with untreated NPC and distant metastasis, treated by radiation schedule in two phsses. In the first phase all patients were treated by conventional radiotherapy with 38Gy on the dacio -neck field, then received 30 -36Gy, the whole dose was 70 -74Gy. Routine fractionated irradiation was made on neck on tangential field. The upperbound of designed therapeutic field was set to connect with lowerbound of main therapeutic field. Patients without lymph node metastasis were given 50Gy to the neck, while 66 -70Gy was delivered to node positive patients. Results：Local completion was observed in 100%. the com- plete response was 93.85%, 1 -year survival rate was 100%, without serious side reacrtion. Conclusion：The perliminary clinical result of late - course 3 - dimensilnal conformal radiation therapy is good.
