摘要
目的观察局部药物注射配合手法松动术治疗肩周炎的疗效。方法治疗组63例,采用2%利多卡因5ml+糜蛋白酶10 000u+0.9%生理盐水15ml关节腔内注射配合关节松动术治疗;对照组63例,采用布洛芬缓释胶囊加功能锻炼。结果治疗组痊愈46例,治疗总有效率为96.83%,对照组痊愈16例,治疗总有效率为58.73%,两组总有效率比较差异有统计学意义(P<0.01)。结论通过以糜蛋白酶为主的药物关节腔内注射,有助于消除炎症,溶解纤维蛋白组织,促进血液及体液的通畅,防止粘连,阻止水肿发展,加速水肿吸收等,同时,配合手法松动肩关节可明显提高临床疗效。
Objective To observe the curative effect of local drug injection combining with manual joint mobilization for treating periarthritis of shoulder. Methods In the treatment group, 63 patients with periarthritis of shoulder were treated by intraarticular injection of medications made up with 1% lidocaine 5 ml and zonolysin 10 000 U and 0. 9 % physiological saline 15 ml combining with joint mobilization for treatment;while in control group, 63 patients with periarthritis of shoulder were treated by buprofen sustained release capsules combining with dirigation for treatment. At the same time, the clinic effect between two groups were observed and compared. Results Forty-six patients were fully recovered from illness in the treatment group and the total effective rate was 96. 83% ;while 16 patients were fully recovered from illness in the control group and the total effective rate was 58.73%. The difference in total effective rate between two groups had statistical significance（P 〈 0. 01 ）. Conclusion Intraarticular injection of zonolysin as main medcation contributes to eliminating inflammation, dissolving fibrous protein tissue, promoting the circulation of blood and body fluid, simultaneously, it will give a help to prevent the development of edema and accelerate the absorption of edema, what is more, it enhances the clinical efficacy noticeably when combining with manual shoulder mobilization.
