摘要
为探讨Q-T离散度(Q-T_d和Q-T_(ed)与凸ΔST/ΔHR指数的关系,观察57例平板运动试验阳性的冠心病患者(冠心病组)和60例运动试验阴性者(对照组)的心电图,比较运动前后Q-T_d和Q-T_(ed)及其与ΔST/ΔHR指数的关系.结果显示:两组运动前Q-T_d(20.35±10.34ms比16.17±10.27ms)、Q-T_(ed)(22.42±11.62ms比17.98±11.62ms)差异均有显著意义(P<0.05);两组运动后Q-T_d(22.11±9.40ms比15.17±94.8ms)、Q-T_(ed)(32.75±14.70ms比25.44±14.87ms)差异均有非常显著意义(P<0.01).冠心病组ΔST/ΔHR指数2.80±1.49μV/次/min,与运动前Q-T_d几和Q-T_(ed)均无关(P>0.05),与运动后Q-T_d和Q-T_(ed)均呈高度正相关(P<0.01).提示运动诱发的心肌缺血是加重心室肌复极不均一的重要因素,Q-T_d、Q-T_(ed)、ΔST/ΔHR指数可作为评定心电图运动试验心肌缺血严重程度的指标.
Purpose of this study wag to assess relationship between Q - T dispersion(Q-Td and Q-Tcd) and △ST/△HR index.Fifty - seven patients with CAD and 60 normal subjects underwent treadmill exercise tests.Q-Td andQ-Tcd before and after exercise tests were measured and relationship to △ST/△HR index was assessed Q-Td and Q -Tcd in C.AD group before exercise test. Results showed that Q-Td and Q -Tcd in CAD group before exercise test were 20.35 ?10.34ms and 22.42 ?1.62ms as against 16.17?10.27ms and 17.98?11.62ms(P< 0.05) in control group. After exercise test, Q -Td and Q -Tcd in these two groups were 22.11?.40ms vs 15.17?.48ms(P<0.01)and 32.75±14.70ms vs 25.44?14.87ms (P<0.01), respectively.△ST/△HR index 2.80 ?1.49 μV/beat/min in CAD group had no bearing on Q -Td and Q - Tcd before treadmill exercise test (P > 0.05) Index did have a direct bearing on Q -Td and Q -Tcd after treadmill exercise tset( P < 0.01) .Conclusion is that exercise-induced ischemia is an important factor to aggravate in homogeneity of ventricular repolarization.Q -Td, Q -Tcd and △ST/△HR indices can be used electrocardiographically to evaluate severity of ischemia during exercise test.
