摘要
目的 评价平板运动试验对冠心病的诊断价值。方法 对 5 2例临床拟诊为冠心病的患者 ,先后行平板运动试验和冠脉造影检查 ,并根据造影结果分为两组 :非冠心病组和冠心病 (CAD)组。结果 (1)CAD组运动中ST段压低较非CAD组显著 (P <0 .0 1) ,前者运动中最大心率 (P <0 .0 0 1)、代谢当量 (Mets) (P <0 .0 5 )低于后者 ;(2 )CAD组△ST/△HR指数、最大ST/HR斜率明显大于非CAD组 (P <0 .0 0 1) ;(3 )平板运动试验ST段压低标准法、△ST/△HR指数法、最大ST/HR斜率法的敏感性分别为 80 %、90 %、92 % ,特异性为 72 .7%、81.8%、76.1%。结论 平板运动试验对冠心病诊断有较高价值 。
Objective To access the clinical value of treadmill exercise test in the diagnosis of coronary artery disease. Methods Treadmill exercise test was performed before coronary angiography(CAG) in 52 patients admitted into our hospital because of suspicion of coronary artery disease,and which were divided into two groups-CAD group and non-CAD group. Results (1) The level of ST segment depression( P <0.01)、△ST/△HR index( P < 0.001 )、maximal ST/HR slope( P < 0.001 ) increased significantly in the CAD group than those in the non-CAD group,but the maximal heart rate( P < 0.001 )and Mets( P <0.05)were less in the former than those in the latter during treadmill exercise test;(2) the sensitivities of ST segment depression、△ST/△HR index、maximal ST/HR slope were 80.0%、90.0%、92.0% respectively,specificity were 72.7%、81.8%、76.2% respectively. Conclusion Combination of ST-segment depression and heart rate during treadmill exercise test can improve the sensitivity and specificity in the diagnosis of CAD.
