摘要
目的观察中心静脉导管闭式引流结合顺铂和苦参碱胸腔内化疗治疗恶性胸腔积液的疗效。方法选择60例恶性胸腔积液患者,随机分为治疗A组、治疗B组和对照组,每组各20例。所有患者采用中心静脉导管胸腔闭式引流后,将顺铂和苦参碱联合注入胸腔,治疗A组:苦参碱注射液150mg+顺铂40mg+NS40ml;治疗B组:苦参碱注射液150mg+顺铂60mg+NS40ml;对照组:顺铂60mg+NS40ml。结果治疗A组和治疗B组疗效、生活质量差异均无统计学意义(P>0.05),治疗A组、治疗B组和对照组疗效、生活质量差异均有统计学意义(P<0.05)。结论中心静脉导管闭式引流联合顺铂和苦参碱腔内化疗治疗恶性胸腔积液是一种安全、方便、有效的方法 ,值得临床推广。
Objective To observe the curative effect of closed thoracic drainage with central venous catheters and combining cis-platinum（DDP）with matrine to pure into the thoracic cavity with malignant pleural effussion.Methods 60 cases with malignant pleural effussion were dividing into three groups：the treated group A were pured 40mg DDP and 150mg matrine into the thoracic cavity with malignant pleural effussion after closed thoracic drainage with central venous catheters,the treated group B were pured 60mg DDP and 150mg matrine,and the control group were pured DDP into the thoracic cavity with malignant pleural effussion after the same treatment.Results There was no significance difference in curative effect and quality of life between the treated group A and B and there is a significant difference in curative effect and quality of life between the treated groups and the control（P〈0.05）.Conclusion The closed thoracic drainage with central venous catheters and combining cis-platinum（DDP）with matrine to pure into the thoracic cavity with malignant pleural effusion is a safe,convenient and effective,which is worthy to be spreaded.
