摘要
目的探讨前后联合入路治疗严重胸腰椎骨折的疗效。方法 2008年10月至2009年9月采用前后联合入路治疗胸腰椎骨折7例。男5例,女2例,年龄35～56岁,平均44.7岁。受伤到手术时间为7h～6d,平均4d,有3例高处跌伤,4例车祸伤,骨折类型按照Magerl分型:A型2例,B1型2例,C2型1例,C3型2例。患者的神经功采用Frankel分级:A级1例,B级2例,C级3例,D级1例。结果手术时间310～480min,平均340min,出血量1200～2500ml(平均1600ml),术后7例获得随访,随访时间7～17个月,平均13.7个月,术后神经功能Frankel分级有1级以上的恢复,术前骨折处后凸角度18.7°±11.3°(3°～28°),术后骨折处后凸角度为0.8°±0.6°(-4°～6°)。结论前后联合入路治疗严重的胸腰椎骨折是一种有效的治疗方法 。
Objective To analyze the results of one-stage combined anteroposterior surgery for serious thoracolumbar vertebral fractures. Methods From March 2008 to September 2009,7 eases of thoracolumbar vertebral fractures were treated with one-stage combined anteroposterior surgery, including 5 males and 2 males aged 35 - 56 years （ mean 42. 6 years）. The time from injury to surgery varied from 7 hours to 6 days, with an average of 4 days. There were 3 cases of falling injuries from height,4 cases of traffic accident injuries. According to Classification of Magerl there were 2 cases of type A3,2 eases of type B1,1 cases of type C2,2 cases of type C3. According to Frankel,there were 1 cases for grade A,2 cases for grade B,3 cases for grade C, 1 cases of grade D. Results The operation time was 310 - 480 minutes （mean 340 minutes）and the blood loss was 1200 2500 nil（ mean 1600 ml）. After operation ,7 cases were followed up for 7 - 17 months with an average of 13.7 months. Neurological status of all patients improved at least 1 Frankel grade at 12 months. The average local kyphosis angle at the time of admission was 18.2. ± 11.3., the local kyphosis angle was reduced to 0. 8. ± 0. 6. immediately after surgery. Conclusion One-stage combined auteroposterior surgery is an effective treatment for serious thoracolumbar fracture.
