儿童声带息肉的病理形态改变与人类乳头瘤病毒的相关性 被引量:1

Correlation of Pathological Changes of Vocal Cord Polyp and Human Papillary Virus in Children

摘要 目的探讨儿童声带息肉组织的形成病因及发生发展机制。方法收集39例儿童声带息肉组织(试验组)及9例尸检正常声带组织中的上皮组织(对照组),测量其上皮厚度与上皮下Reinke′s间隙厚度。运用原位杂交方法检测2组人类乳头瘤病毒(HPV)6/11 DNA,免疫组织化学方法检测其P63、鼠双微基因2(MDM2)蛋白的表达。结果试验组声带息肉组织中,上皮无角化者19例,角化不全者11例,完全角化者9例;上皮呈内生型生长者15例,呈外生型生长者10例,呈混合型生长者14例。试验组上皮厚度最高值及平均值、Reinke′s间隙厚度分别为(601.03±229.99)μm、(390.43±158.36)μm、(28.07±14.00)μm,HPV6/11 DNA的阳性表达率为48.72%,P63、MDM2蛋白的阳性表达值分别是33.62±16.91、153.93±11.59。对照组上皮厚度的最高值和平均值、Reinke′s间隙厚度分别为(152.95±16.17)μm、(135.30±24.04)μm、(11.26±2.66)μm,HPV6/11 DNA的阳性表达率为14.29%,P63、MDM2蛋白的阳性表达值分别是8.75±4.57、163.73±6.01。试验组各项指标与对照组比较,差异均有统计学意义(Pa<0.01,0.05)。HPV6/11的阳性表达与儿童声带息肉上皮组织的最高厚度、平均厚度均呈正相关(r=0.583、0.520,Pa<0.05)。结论 HPV6/11感染是儿童声带息肉形成的重要原因之一。P63、MDM2在儿童声带息肉形成过程中发挥重要作用。 Objective To explore the cause and mechanism of vocal cord polyps in children.Methods Thirty-nine cases of vocal cord polyps of children(experimental group) and 9 cases of normal autopsy(control group) were collected,and the differences of pathological changes,the thickness of epithelial tissue and Reinke′s clearance,levels of human papillary virus(HPV)6/11 DNA of in situ hybridization and an immunohistochemical panel of P63,murine double minute 2(MDM2) protein were compared between the 2 groups.Results In the experimental group,the thickness of tallest value,average value of epithelial tissue and Reinke′s clearance were(601.03±229.99) μm,(390.43±158.36) μm and(28.07±14.00) μm,respectively,while the expression rate of HPV6/11 was 48.72%,and the expression values of P63 and MDM2 were 33.62±16.91 and 153.93±11.59,respectively.In control group,the thickness of tallest value,average value of epithelial tissue and Reinke′s clearance were(152.95±16.17) μm,(135.30±24.04) μm and(11.26±2.66) μm,respectively,while the expression rate of HPV6/11 was 14.29%,and the expression values of P63 and MDM2 were 8.75±4.57 and 163.73±6.01,respectively.Compared with the expression of each marker between the 2 groups,the differences all had statistically significant(Pa〈0.01,0.05).The expression of HPV6/11 was related with the thickness of tallest value and average value of epithelial tissue closely(r=0.583,0.520,Pa〈0.05).Conclusion The infection of HPV6/11 and P63,MDM2 play an important roles in the development of children′s vocal cord polyps.
