摘要
目的:观察阴道菌群密集度和多样性在BV、BV过渡态及健康妇女间的差异;乳酸杆菌及产H2O2乳酸杆菌在BV发展过程中数量变化。评估乳酸杆菌、产H2O2乳酸杆菌与BV关系;阴道菌群密集度和多性与微生态诊断法的差异及在阴道微生态诊断中的价值。方法:Amsel、Nugent评分及培养法,对200例BV患者及100例对照组阴道分泌物进行Nugent评分分级及线索细胞阳性比率测定,对不同Nugent评分进行乳酸杆菌及产H2O2乳酸杆菌数量和阴道菌群密集度、多样性进行观察统计。结果:健康妇女阴道菌群密集度主要为2级,过渡态主要为1级,而BV则集中在3级和4级;多样性主要为1级、过渡态为1级和2级,而BV主要集中在2级和3级。健康组主要以乳酸杆菌计数4+为主,Nugent评分诊断BV过渡态在健康组和BV患者组间存在显著差异。Nugent评分7~10分,健康对照组和BV组的乳酸杆菌几乎消失。健康组中产H2O2乳酸杆菌计数4+病例占55.0%,Nugent评分4~6分和7~10分,产H2O2乳酸杆菌计数在健康组和BV患者组间均显著减少或消失。结论:阴道菌群密集度、多样性与微生态诊断法有显著差别,阴道微生态有其独特的菌群组成结构,对BV微生态评价应建立独立的阴道微生态标准进行。乳酸杆菌、而不仅是产H2O2乳酸杆菌数量的急剧下降是BV发生和发展的基础;乳酸杆菌的总量比产H2O2乳酸杆菌数量在维持阴道微生态稳定方面更重要。
Objective:To observe discrepancy of vagina flora density or diversity among BV transition state and normal state;the relation of Lactobacillus or hydrogen peroxide producing Lactobacillus and BV;To evaluate the difference between micro-ecology diagnosis and vagina flora density or diversity.Methods:To extract the Vaginal discharge of 200 BV patients and 100 no BV patients.To observe Clue cell ratio and quantity change of Lactobacillus/hydrogen peroxide producing Lactobacillus with Nugent score and vagina flora density or diversity by Asmel,Nugent score and germiculture.Results:Health group women vagina flora density was mainly 2 degree,transient state group was 1 degree but BV was 3 or 4 degree.While health group women vagina flora diversity was mainly 1 degree,but BV was 2 or 3 degree.The rate of Lactobacillus 4+ in health group was 77.3%,transient state between health and BV groups was significantly difference.Lactobacillus almost vanished between health and BV groups when Nugent score was 7~10.The rate of hydrogen peroxide producing Lactobacillus 4+ in health group was 55.0%,no hydrogen peroxide producing Lactobacillus in health group was almost 20%.But hydrogen peroxide producing Lactobacillus almost vanished between health and BV groups when Nugent score was 4~10.Conclusion:There is significant difference between vagina flora density or diversity and micro-ecology diagnosis.Vagina micro-ecology has unique flora composition and should build up specialty vagina micro-ecology criterion to evaluate BV.Quantitative toboggan of Lactobacillus but hydrogen peroxide producing Lactobacillus is the base of BV.The number of Lactobacillus plays a key role in maintaining micro-ecology stabilization than the quantity of hydrogen peroxide producing Lactobacillus.
