摘要
目的:观察DSA引导下射频热凝术治疗原发性三叉神经痛的近、中期临床疗效及安全性。方法:27例原发性三叉神经痛患者,在DSA引导定位下行三叉神经半月节射频热凝治疗。治疗前后进行VAS评分,观察术后近期临床疗效、不良反应,并对中期疗效进行随访。结果:所有患者均穿刺成功。射频热凝术后全部患者疼痛缓解,术后1d、3d、1周VAS评分分别为2.12±0.64、1.04±1.02、0.46±0.31,较治疗前均有显著降低(P<0.01);优良率分别为55.6%、74.1%、92.6%。无严重不良反应发生。术后1年随访22例,优良率86.4%,1例复发,占4.5%。结论:DSA引导定位下射频热凝术治疗原发性三叉神经痛是一种有效、安全的治疗方法。
Objective：To observe the clinical efficacy and safety of radiofrequency thermocoagulation on the treatment of primary trigeminal neuralgia guided with digital substraction angiography（DSA） in short-term,mid-term prospect.Methods：Twenty-seven patients of PTN were positioned by DSA,then treated with radiofrequency thermocoagulation.The VAS scores,clinical efficacy and complication were observed nearly post-treatment.The effects in mid-term was assessed in secondary follow-up.Results：All patients were punctured successfully.After treatment,all patients have pain relief.VAS scores on day 1,day 3,week 1 after radiofrequency thermocoagulation were 2.12±0.64,1.04±1.02,0.46±0.31 respectively,decreased compared with that before treatment（P0.01）,and the satisfactory rate were 55.6%,74.1%,92.6% respectively.No serious complication occurred in all of the patients.22 patients were invovled in the follow-up survey 1 year after radiofrequency thermocoagulation,and the satisfactory rate was 86.4%,1 patient recurred,the recurrence rate was 4.5%.Conclusion：Radiofrequency thermocoagulation guided with DSA for the treatment of primary trigeminal neuralgia is effective and safe.
