摘要
目的评价凉血解毒汤治疗白疕血热证的临床疗效、安全性及对血清炎症因子所产生的影响。方法对41例患者采用随机、单盲(病人)、对照的研究方法。治疗组(18例)采用凉血解毒汤,对照组(23例)采用医师自拟方,分别治疗8周,进行疗效判定及安全性检测;采用放免法测定治疗前后银屑病患者血清白介素6、肿瘤坏死因子α水平,采用双抗体夹心ELISA法测定血清白介素2、白介素8、可溶性白细胞介素2受体、干扰素含量。结果治疗后两组皮损的PASI评分均较治疗前降低,两组患者临床疗效比较无统计学差异(P>0.05),治疗组总有效率为76.5%,对照组总有效率为82.6%;两组均未见确切的不良反应。两组治疗后较治疗前IL-6、TNF-α、sIL-2R、IL-8、IFN-γ水平均降低,与治疗前比较,差异有统计学意义(P<0.05或P<0.01),两组治疗后较治疗前IL-2水平均降低,但两组内和组间差异均无统计学意义(P>0.05)。结论凉血解毒汤对寻常型银屑病血热证安全有效,而且能够下调IL-6、TNF-α、sIL-2R、IL-8、IFN-γ水平。
Objective To evaluate the clinical effect and safety as well as the impacts on serum inflammatory factors in treatment of white facial skin disease（psoriasis vulgaris）of blood heat syndrome with cooling blood and removing toxin decoction（liangxue jied decoction,LXJDD for short）.Methods The randomized,single-blind and controlled study was applied to 41 cases.The treatment group（18 cases）was treated with LXJDD and control group（23 cases）with self-prescribed formula.The treatment lasted for 8 weeks respectively and the efficacy and safety were assessed.The radio-immunity method was adopted to detect the levels of IL-6 and TNF-α in serum in psoriasis patients before and after treatment.The enzyme-linked immunosorbent assay-sandwich technique was adopted to detect the levels of IL-2,IL-8,sIL-2R and IFN in serum.Results PASI score reduced in both groups after treatment as compared with that before treatment,and there was no statistical difference in clinical efficacy between two groups（P〉0.05）.The total effective rates were 76.5% and 82.6% respectively in treatment group and control group.Moreover,there was no exactly adverse effect in both groups.The levels of IL-6,TNF-α,sIL-2R,IL-8 and IFN-γ dropped after treatment in both groups as compared with those before treatment,indicating significant difference in statistic（P〈0.05 or P〈0.01）.IL-2 level decreased in two groups after treatment as compared with that before treatment,but without significant statistical difference indicated（P〉0.05）.Conclusion Liangxue Jiedu decoction is safe and effective on psoriasis vulgaris of blood heat syndrome and can down-regulate the levels of IL-6,TNF-α,sIL-2R,IL-8 and IFN-γ.
