高敏C反应蛋白对高血压前期人群进展至高血压的预测价值 被引量:57

Risk Prediction of High Sensitivity C-reactive Protein on The Incidence of Hypertension in Prehypertensive Population

摘要 目的探讨血清高敏C反应蛋白(hsCRP)水平对高血压前期人群进展至高血压的预测价值。方法此研究为前瞻性队列研究,对象来自2006-2007年度河北唐山开滦集团公司员工健康查体数据库,选取符合高血压前期诊断标准的研究对象作为观察队列,共33 913人,于2008-2009年度进行第二次体检,纳入最终统计分析的有效数据为24 441例(男性19 345例,女性5096例),年龄19~93岁。用多元线性回归分析基线lg hsCRP水平对第二次体检时收缩压、舒张压的影响。按基线hsCRP水平四分位数分组,用Logistic回归分析基线hsCRP水平对高血压前期人群进展至高血压的预测价值。结果①2年间观察队列中有8026例进展至高血压,进展至高血压的比例为32.8%。②多元线性回归分析显示,基线lg hsCRP每增加1个单位,收缩压增加0.39 mm Hg(P<0.01)。③按基线hsCRP四分位数分组,用多因素Logistic回归分析校正基线年龄、性别、三酰甘油、总胆固醇、低密度脂蛋白胆固醇、空腹血糖、收缩压、舒张压、腰围后,hsCRP仍是进展至高血压的独立危险因素,最高四分位数组进展至高血压的风险是最低四分位数组的1.10(95%CI1.02~1.19;P<0.05)倍。结论hsCRP是高血压前期进展至高血压的独立危险因素,对高血压前期进展至高血压具有一定的预测价值。 Objective To investigate the prognostic significance of high sensitivity C-reactive protein(hsCRP) on the incidence of hypertension in prehypertensive population.Methods The subjects for this community-based prospective cohort study were recruited from the Kailuan Company in 2006-2007,consisted of 33 913 prehypertensive indivuduals.After 2-years follow-up,data of 24 441 subjects(19 345 males and 5096 females) aged 19-93 were collected their data available.Multiple linear regression analysis was used to analyze the influence of baseline lg hsCRP on blood pressures at the end of follow-up.Logistic regression analysis was used to analyze the prognostic significance of hsCRP on the incidence of hypertension in prehypertensive population.Results ①8026 subjects(32.8%) developed into hypertension after 2 years follow-up.②Multiple linear regression analysis showed that systolic blood pressure increased 0.39 mm Hg with each one unit increase of baseline lg hsCRP(P〈0.01).③Logistic regression analysis showed that hsCRP is an independent risk factor for hypertension in prehypertensive population after adjusting of baseline age,gender,TG,TC,LDL-C,FBG,SBP,DBP and waist circumference.The risk of development of established hypertension in those individuals with the highest quartile of hsCRP at baseline was higher than those with the lowest quartile of baseline hsCRP(RR=1.10,95% CI 1.02-1.19;P〈0.05).Conclusion The hsCRP is an independent risk factor of hypertension and might be used to predictive the risk of development of establised hypertension in prehypertensive population.
