摘要
目的应用眼反应分析仪(ORA)观察准分子激光原位角膜磨镶术(LASIK)前后角膜生物力学特性的改变,并对其影响因素进行分析。方法对20例(38只眼)近视及近视散光患者行常规LASIK手术,于术前及术后1周至1个月用ORA测量角膜滞后量(Corneal Hysteresis,CH)和角膜阻力系数(Corneal Resistance Factor,CRF)。结果CH、CRF术前为(9.92±1.27)mmHg、(10.96±1.53)mmHg术后为(7.89±1.16)mmHg及(7.63±1.21)mmHg,术前后比较差异明显有统计学意义(P=0.000)。术前CH、CRF与中央角膜厚度呈正相关(r=0.509,P=0.001;r=0.477,P=0.002),其变化量△CH、△CRF与切削深度呈正相关(r=0.543,P=0.000;r=0.574,P=0.000)。结论LASIK术后角膜生物力学特性发生显著变化,术后CH、CRF及其变化量与切削深度、切削比等因素有关。
Objective To observe and evaluate the influence of LASIK on corneal biomechanical properties by using ORA (ocular response analyzer).Methods Thirty-eight eyes were evaluated by using ORA pre- and post LASIK.The values of CH and CRF were recorded and analyzed statistically by SPSS 13.0. Results Compared with the values before LASIK, CH and CRF decreased sharply postoperatively.CH and CRF showed moderate correlation to CCT and RSB.There was a trend toward positive correlation between AD and △CH, △CRF, respectively (r=0.543, P=0.000; r=0.574, P=-0.000).Conclusions LASIK substantially weakened corneal tensile strength and corneal viscoelastic properties.The good correlation between AD and the changes of CH, CRF in LASIK, implies that the amount of ablation is more sensitive to the changes of corneal postoperative biomechanical parameters.
