摘要
目的:探讨腹部不留瘢痕腹腔镜胆囊切除术的手术方法。方法:沿脐上缘呈"八"字形做两个5mm切口,置入5mm腹腔镜和胆囊抓钳;于脐下缘做10mm切口,置入常规器械,并由此孔取出胆囊。结果:16例手术均顺利完成,1例因炎性渗出较多,于右侧肋缘下另做戳孔放置引流管,无中转开腹及胆漏、出血等并发症发生;手术时间30～55min,平均44min。术后住院2～3d,脐部切口愈合良好,无可见瘢痕。结论:经脐腹腔镜胆囊切除术安全可行,可达到腹部无可见瘢痕的目的。
Objective：To explore the operative method of laparoscopic cholecystectomy （LC） without any abdominal scars.Methods：A 10mm splay incision was made on the upper edge of umbilicus,5mm laparoscope and gallbladder grasping forceps were inserted.A 10mm incision was made at the lower border of umbilicus,from which regular operative equipment was inserted and the gallbladder was removed.Results：Sixteen operations were successful.Because of too many inflammatory exudates,an incision was made in the right lower margin of the ribs to place drainage tube in one patient.No conversion to open operation was required,and no complications such as bile leakage or bleeding occurred.The operating time was 30-55min,with an average of 44min.Postoperative hospital stay was 2-3d,and umbilical incision healed well without visible scar.Conclusions：Transumbilical LC is safe and feasible,and can achieve the purpose of invisible scar at abdomen.
