摘要
目的：探讨常规腹腔镜器械经脐单孔腹腔镜胆囊切除术的安全性及临床效果。方法：回顾分析我院2010年11月-2011年8月间开展腹腔镜胆囊切除术213例患者的临床资料，将其中使用常规腹腔镜器械经脐单孔腹腔镜胆囊切除术的61例患者作为A组，使用传统腹腔镜胆囊切除术的152例作为B组，对比两组手术时间、术中出血量、手术并发症、术后疼痛程度、术后进食时间、下床活动时间、住院时间及住院总费用等指标。结果：两组均痊愈出院，无出血、胆漏、胆管损伤等并发症发生。术中出血量及并发症，术后进食时间，下床活动时间两组无差异（P〉0．05）；但A组术后疼痛较轻，住院时间及住院总费用低于B组（P〈0．05）。结论：只要适应证选择恰当，使用常规腹腔镜器械经脐单孔腹腔镜胆囊切除术与传统腹腔镜胆囊切除术一样安全可行，美容效果及病人的心理满意度较好。
Objective：To discuss the safety and clinical effect of transumbilical single port laparoscopic cholecystectomy by the routine instruments. Methods：Retrospective analysis study of 213 patients was achieved who undergone laparoscopic cholecystectomy from Nov. 2010 to Sep. 2011. 61 patients who undergone transumbilical single port laparoseopic cholecystectomy by roatine instruments consisted of group A, group B included 152 patients who undergone conventional laparoscopic cholecystectomy. The length of operation time, blood loss, complications rate, the pain level, the time for taking food, activity recovery time after operation, hospital stay after operation and the total cost were analyzed between two groups. Results：All operations were successful, post- operative complications such as bleeding, bile leakage and injury of bile duct did not occur in both groups. As to the blood loss, complications and activity recovery time after operation, group A and group B had no significant difference（P 〉0.05）. The pain level, post -operative hospital stay and the total hospital cost in group A were superior to group B（ P 〈 0.05 ）. However, the indications of group A were fewer than group B. Conclusions ：Transumbilical single port laparoscopic cholecystectomy by ordinary instruments is as safe and feasible as conventional laparoscopic cholecystectomy.
