早期肠内肠外联合营养对机械通气慢性阻塞性肺疾病患者的影响 被引量:5

Effect of enteral nutrition combined with parenteral nutrition supporting at early stage on patients with chronic obstructive pulmonary disease undergoing mechanical ventilation

摘要 目的:观察早期肠内肠外联合营养对慢性阻塞性肺病(COPD)患者的机械通气时间、胃肠功能、营养状况的影响。方法:28例入住ICU需机械通气的慢性阻塞性肺病患者,随机分为两组,分别给予早期肠内与肠外联合营养支持(EN+PN组,13例)及肠内营养支持(EN组,15例)。观察营养支持观察两组间胃肠功能情况及患者机械通气时间及预后情况。结果:1周内,EN+PN组患者13例未发生胃肠道不耐受症状;EN组15例发生腹胀及胃潴留5例,腹泻4例。1周后,EN+PN组患者血清白蛋白水平较营养支持前有明显升高,差异有统计学意义(P<0.05)。EN+PN组患者成功撤机10例(76.92%),机械通气时间为(9.4±2.6)d,EN组患者成功撤机11例(73.33%),机械通气时间为(11.6±1.8)d。两组成功撤机例数比较无差异,EN+PN组机械通气时间明显短于EN组,差异有统计学意义(P<0.05)。结论:肠内肠外联合营养支持较完全肠内营养支持的胃肠耐受性良好,能较好的保证机械通气的慢性阻塞性肺病患者营养需求,缩短机械通气时间。 Objective To observe the time in undergoing mechanical ventilation, gastrointestinal function, nutritional state of patients with chronic obstructive pulmonary disease(COPD) undergoing mechanical ventilation supported by enteral nutrition combined with parenteral nutrition at early stage. Method 28 patients with COPD undergoing mechanical ventilation in ICU were divided into EN+PN group (n=13) and EN group (n=15) randomly, supplied with enteral nutrition combined with parenteral nutrition at early stage and enteral nutrition respectively. Result All patients in EN+PN group hadn' t gastrointestinal intolerance in the first week. There were 5 patients suffered from abdominal distention and gastric retention, and 4 patients suffered from diarrhea in EN group. After one week, serum-albumin of patients in EN+PN group were higher than those before nutritional support (P〈0.05). There wasn' t different between successful rate of mechanical v^ntilation weaning of two groups, but the time in mechanical ventilation of EN+PN group was shorter than that of EN group (P〈0.05). Conclusion Enteral nutrition combined with parenteral nutrition had better gastrointestinal tolerance than enteral nutrition, and supplied patients nutritional support sufficiently, decreased the time in undergoing mechanical ventilation.
