慢性宫颈炎的病毒病原研究和重组人α1b型干扰素的治疗作用

摘要 阐明人乳头瘤病毒16型(HPV-16)、巨细胞病毒(CMV)和单纯疱疹病毒2型(HSV-2)感染与慢性宫颈炎的关系,考核重组人α1b型干扰素治疗慢性宫颈炎的疗效和阻断HPV-16母婴传播的作用。方法:采用DNA分子杂交技术、病毒分离技术,PCR扩增技术检测标本。标本采自糜烂、正常宫颈,妊娠39周的孕妇宫颈以及新生儿咽拭子。结果:通过对正常宫颈和糜烂宫颈HPV-16,HSV-2和CMV病毒检出率的比较,证明病毒感染与慢性宫颈炎高度相关;采用重组人α1b型干扰素(rhIFN-α1b)治疗慢性宫颈炎一个疗程后,有效率达938%,显效率达60%;HPV-16和HSV-2的病毒检出率也相应明显下降。在干扰素治疗后,HPV-16的母婴传播率从80%下降至294%。中药黄芪与干扰素有协同作用。结论:1)慢性宫颈炎与HPV-16,HSV-2和CMV病毒感染有关;2)rhIFN-α1b对治疗慢性宫颈炎有明显疗效;对阻断HPV-16的母婴传播有明显作用; Objective:To demonstrate the chronic cervicitis being related to papilomavirus type 16(HPV-16),herpes simplex virus type 1 (HSV-2) and cytomegalovirus (CMV) infections ,and to observe the clinical efficacy of a recombinant human interferon alpha 1b (rhIFN-α1b)in treatment of chronic cervicitis and blocking the perinatal transmission of HPV-16. Methods:Using DNA hybridization technique,virus isolation and PCR method from samples taken from erosive,normal cervices as well as cervices of pregnant women with 38th gestational weeks. Results: The chronic cervicitis was shown to be highly related to HPV-16,HSV-2 and CMV infections as demonstrated by comparision with virus detection rate from erosive and normal crvices ;after one course of treatment with rhIFN-α1b was also to effective on block the perinatal transmission of a HPV-16,the perinatal transmission rate dropped down from 80% to 29.4% after treatment.Astragalus membranaceus(AM),a Chinese herbal durg,showed a synergic effect on interferon therapy. Conclusion: 1) Chronic cervicitis is closely related to viral infections; 2) A recombinant human interferon alpha 1b is very effective to treat the chronic cervicitis and block the perinatal transmission of HPV-16; 3)AM has a synergism to interferon therapy.
