摘要
目的:检测甲状腺髓样癌内凋亡细胞、bcl-2、p53和Ki-67,探讨其与患者的预后关系。方法:在21例甲状腺髓样癌内,用DNA末端标记方法检测凋亡细胞和用免疫组织化学技术检测bcl-2、p53及Ki-67的表达。结果:在21例甲状腺髓样癌标本中,凋亡细胞bcl-2、p53和Ki-67的检出率分别为81%,100%,62%和71%。bcl-2阳性物质位于肿瘤细胞的胞浆内,少数病例则位于膜上。p53蛋白阳性主要位于胞核内,少数病例也同时见于胞浆内。Ki-67抗原主要位于肿瘤细胞核内。本研究结果表明,bcl-2和肿瘤细胞凋亡之间有密切关系(P<0.01)。bcl-2阳性细胞检出率较高的病例,凋亡细胞检出率则较低;反之,凋亡细胞检出率则较高。p53和Ki-67阳性细胞检出率和细胞凋亡无相关性(P>0.05)。结论:bcl-2具有抑制细胞凋亡的作用。上述各因素检测结果与患者存活率无统计学意义。bcl-2肿瘤蛋白在甲状腺髓样癌中高表达,可作为该肿瘤的一个新的标记物。
Purpose To explore the prognosis of patients with medullary thyroid carcinoma (MTC), the apoptotic cells, bcl 2, p53 and Ki 67 in 21 cases of human medullery thyroid carcinoma were detected. Methods Detection of apoptotic cells was used by DNA nick end labeling method and detection of bcl 2, p53 and Ki 67 by immunohistochemical ABC method. Results Among the 21 cases of medullary thyroid carcinoma, the detection rate of apoptotic cells, bcl 2, p53 and Ki 67 were 81%, 100%, 62% and 71%, respectively. bcl 2 positive substance located in the cytoplasm of MTC, in a few cases in the nuclei, p53 oncoprotein located mainly in the nuclei of MTC, but that was found in the cytoplasm in a few of cases. Ki 67 antigen was mainly located in the nuclei of MTC. In the cases of higher detection rate for bcl 2 positive cells, the detection rate of apoptotic cells was lower, conversely, the detection rate of apoptotic cells was higher. The result suggested that there is a close relationship between bcl 2 and cell apoptosis ( P< 0 01). The correlation between cell apoptosis and detection rate of p53 and Ki 67 positive cells was not significant ( P> 0 05). Conclusion bcl 2 expression has been shown to inhibit cell apoptosis. The correlation between the cell apoptosis, bcl 2, p53 and Ki 67 and ratio of patien survival was not statistical significance. The overexpression of bcl 2 oncoprotein in MTC could be a marker in predicting the outcome of the disease.
